Abstract

We found that there is insu icient evidence to establish whether denser or normal bupivacaine is the more e ective in reducing the need for: a) switching to general anaesthesia; b) additional pain relief medication. Denser bupivacaine had a more rapid onset of pain relief. Quality of evidence: Due to the di erences among the included studies in doses given, variety of additional drugs used for pain relief, variation in regional anaesthesia techniques and the small numbers of participants, we rated the overall quality of evidence for di erent outcomes from very low to moderate, as very few women experienced untoward events in all trials. “…The remaining 12 articles did not fulfil our inclusion criteria and were excluded for the following reasons: three out of the 12 excluded studies used hypobaric bupivacaine group ; one used mepivacaine ; two studies used tetracaine instead of bupivacaine ; three studies used combined spinal‐epidural technique with the addition of local anaesthetics into epidural space (although we only extracted data from the patients who had spinal anaesthesia without epidural injection); one study used differing concentration of isobaric and hyperbaric bupivacaine ; whereas two studied employed sequential administration of isobaric and hyperbaric bupivacaine . Efficacy of fentanyl combined with bupivacaine and morphine for spinal anesthesia during cesarean section: A double-blind randomized controlled trial

“…To prevent traction reaction and pain, a higher anesthesia block level of T4 is necessary. 33,34 When the block reaches the level of T4, thoracic spinal nerve block can occur, resulting in paralysis of the intercostal muscles and respiratory depression. 35 The potential for fetal exposure to opioids administered via SA is a valid concern.…”
