Human parvovirus B19 infection in HIV-positive patients

Abstract: Parvovirus B19 infects predominantly erythroid cells, leading to transient inhibition of erythropoiesis. Immunocompromised patients may be unable to produce neutralizing antibodies and may develop severe chronic anemia. Epidemiological studies done on Niterói population showed that B19 infection occurs periodically in late spring and summer. We report a study from 55 HIV infected patients attending an infectious diseases outpatient clinic in this city during a 5-month period in which B19 circulation was well d… Show more

“…His blood counts were unexceptional, except for a relatively low hematocrit in January 1999 and the increased medium corpuscular volume commonly observed in patients taking zidovudine. This patient was among an HIV-positive cohort followed by us in a previous study 1 , in which he had been repeatedly found reactive for serum IgG against parvovirus B19. On that occasion (November 1997) his serum dot blot and IgM against parvovirus B19 were both negative.…”

“…The prevalence and severity of anemia increase as the HIV infection advances to AIDS 21 . Several causes of anemia have been described in HIV patients such as drugs (zidovudine, ganciclovir, trimethoprim-sulfamethoxazole), lymphomas, direct effect of HIV on bone marrow cells and co-infection with mycobacteria, malaria or B19V 27 . In this study we excluded HIV patients on HAART because of the influ-ence some of these drugs may have on the status of anemia in the individual.…”

“…However, Patients with a CD4 count greater than 300 cells/µl have been shown to be capable of producing neutralizing antibodies. Persistent infections are also more common among patients with advanced immunodeficiency 27 . Therefore, detecting IgG may not be definitive of persistent infection, considering the impaired ability to produce neutralizing antibodies against B19V especially in some HIV patients 21 .…”

“…Immunoglobulin treatment can cure the infection and, sometimes and paradoxically, transiently worsen the clinical picture by triggering immune complex symptoms (Frickhofen et al 1990, Setúbal et al 2003, Young & Brown 2004). Since 1998 we have been studying erythrovirus B19 infection in our HIV infected patients in Hospital Universitário Antônio Pedro (HUAP), Niterói, Rio de Janeiro, Brazil (Aguiar et al 2001, Setúbal et al 2003. We have always considered co-infection infrequent and attributed this to the fact that most of our HIV infected patients were under anti-retroviral therapy.…”
