How to Perform and Interpret a High-resolution Anorectal Manometry Test

Abstract: High-resolution anorectal manometry (HR-ARM) and high-definition anorectal manometry (HD-ARM) catheters have closely spaced water-perfused or solid state circumferentially-oriented pressure sensors that provide much better spatiotemporal pressurization than non-high resolution catheters. This is a comprehensive review of HR-ARM and HD-ARM anorectal manometry catheter systems, the methods for conducting, analyzing, and interpreting HR-ARM and HD-ARM, and a comparison of HR-ARM with non-high resolution anorectal… Show more

“…These maneuvers are indicated to assess the integrity of spinal reflex pathways of the patients; normally, the increased abdominal pressure triggers voluntarily external sphincter contraction. 26 As mentioned above, the complexity of anatomical perineal structures contribute also to the anal tonus. For that reason, voluntary anorectal manometry maneuvers are important for the rationale of biofeedback therapy in patients presenting a severe incontinence due to technically surgical problems.…”

“…26 Therefore, a reduced anal resting sphincter pressure could be explained by an anal sphincter deficit, especially in HD patients who underwent a pull-through procedure. In the literature, several authors stated that patients with true incontinence present a significantly lower resting pressure than normal subjects.…”

“…However, the design of rectal sensors is different from those placed in the anal canal in most catheters. Furthermore, the rectal sensors are generally covered by a non-latex balloon, which impacts on the validity of measurement of rectal pressures [19].…”

“…Anorectal manometry (ARM) is used to diagnose defecatory disorders and identify anal weakness in fecal incontinence (FI) (13). While high resolution manometry (HRM) catheters provide better spatial resolution, the pressures recorded with ManoscanTM HRM catheters vary over time (4, 5).…”
