Background: Prostatic diseases are a leading cause of frequent urinary complaints in elderly males. The most common diseases include prostatitis, benign prostatic hyperplasia, and prostate cancer. This study aimed to determine the frequency of the prostatic diseases, their trend over the years, and the association of age and diagnosis. Methods: A retrospective descriptive study was conducted in Basic Medical Sciences Institute (BMSI) at Jinnah Postgraduate Medical Centre, Karachi. Data was collected from the histopathological files and all the cases diagnosed from 2014 to 2018 were included. Statistical analysis was done using SPSS with 95% confidence interval and p-value of ≤ 0.05 was considered significant. The Chi-square test was applied to find the association with age and year of diagnosis. Results: Total 163 prostate cases were documented from 2014 to 2018, out of which 98 (60.1%) were of benign prostatic hyperplasia, which was most commonly diagnosed in men aged, 61-70. Prostatic adenocarcinomas having Gleason scores 8-10 were frequent and commonly seen in men aged 51-60 years. An increasing trend (p-value=0.053) was observed in the diagnosis of these prostatic diseases within these five years. Conclusion: Benign prostatic hyperplasia is the most frequently (p-value=0.140) diagnosed prostatic disease in men, followed by adenocarcinoma and prostatitis. Advanced stage prostatic adenocarcinoma was diagnosed more frequently 17(10.4%) and at an earlier age. There is an increase in the number (p-value=0.053) of cases of prostatic diseases diagnosed each year. National-level studies and health policies are needed to facilitate early diagnosis and treatment.

Objective: To assess the frequencies of various histopathological pattern of prostatic diseases in a tertiary care hospital at Karachi. Study Design: Cross Sectional Observational. Setting: Department of Pathology, Pakistan Navy Station Shifa Hospital, Karachi. Period: January 2018 to February 2019. Material & Methods: Our study comprised of 160 Prostatic specimen including prostatectomies and Transurethral resection of prostate (TURP). All malignancies were graded according to Modified Gleason score for diagnostics and to calculate the frequency. Data was evaluated by using SPSS version 23. All frequencies were determined in terms of percentages. Results: A total of 160 prostatic samples comprised of 123 (76.87%) benign and 37(23.12%) malignant cases. Both transuretheral prostatectomies (84.4%) and radical prostatectomies specimens (15.6%) were included in the study Benign cases were diagnosed as only benign prostatic hyperplasia (BPH) 93.49%. Moreover, additional findings observed in the benign cases alongside benign prostatic hyperplasia were Acute/chronic prostatitis in 4% samples, granulomatous prostatitis in 1.6% cases and eosinophilic prostatitis in 0.8% case. In the malignant cases, majority of the cases were of prostatic acinar adenocarcinoma n=35(94.6%) while 2(5.4%) cases exhibited transitional cell carcinoma, perineural invasion was seen in 10 cases. Conclusion: Hyperplasia of prostate was reported in majority of the cases in our set up the most individuals were in the age group was 61-70 years. Adenocarcinoma of the prostate, was less commonly diagnosed in the age group of 51-60 years.

Objectives: To determine the pattern of histological findings in prostatic chips obtained from cases of enlarged prostate gland and to correlate the findings with PSA levels. Materials and Methods: It was a cross sectional descriptive study conducted in Northwest General Hospital , Peshawar from 1st January 2018 to 23rd December 2021. A total of 500 male cases with features of enlarged prostate, frequency, urgency and dribbling with mean age of 52 ± 12.3 years (range 45 -80 years) were included in the study. The prostatic biopsy specimens were obtained and PSA levels were done in all cases. The histological specimens were subjected to microscopic examinations and findings were recorded in proforma. The findings were correlated with the PSA levels. Mean and standard deviation were used for quantitative data. Frequency and percentages were used for qualitative data Results: Out of 500 cases of enlarged prostate, benign prostate hypertrophy was seen in 375 (75%) cases. 125 cases (25%) had micro invasive carcinoma prostate . All 375 cases of benign prostate hypertrophy had prostate specific antigen levels less than 4ng. Out of 125 cases of micro invasive carcinoma, 120 (96%) had PSA levels >10ng . Sensitivity of PSA at levels of 10ng/ml for prostatic carcinoma was 96%. Conclusion: Benign prostate hypertrophy is the commonest lesion in cases of prostate pathology. PSA is useful marker to determine the presence or absence of micro invasive carcinoma in patients with prostate hypertrophy.
