Purpose Non-obstructive azoospermia (NOA) is one the many causes of male infertility (10 %) resulting from testicular failure. Multiple testicular biopsies fail to find mature sperm in at least 50 % of cases Therefore; hunting for sensitive and specific biomarkers of spermatogenesis that could better determine the fertility status in NOA can lead to improved management of male infertility. Therefore, we evaluated sperm production through analyses of germ cell-specific transcripts (DAZ, TSPY1, SPTRX3 and SPTRX1) in semen and testicular biopsies of men with azoospermia. Methods We collected semen (N=83) and testis biopsies (N= 31) from men with non-obstructive azoospermia. We later extracted RNA and synthesized cDNA using washed semen precipitate and testicular tissues. We also performed seminested PCR with designed specific primers. Using H&E method, an expert pathologist performed the histopathological evaluation. Having categorized the patients into three groups based on histopathological results, we calculated the agreement between molecular results of semen and tissues with histopathological findings for each patient using Kappa statistical test. Results Molecular findings of precipitated semen and testicular tissues were in disagreement with histopathological results in most cases. Molecular analysis of testis biopsies showed significant difference (Kappa coefficient=0.009, P value= 0.894) with histopathological results; TSPY1, DAZ, SPTRX3 and SPTRX1 were respectively detected in 94 %, 94 %, 17.6 % and 52.9 % of men diagnosed with germ cell aplasia.Capsule The molecular analysis of testicular tissues and semen precipitates for the presence of germ cell-specific transcripts is a diagnostic test that could be of help in the detection of active spermatogenesis in men with non-obstructive azoospermia.

RESUMELes progr~s r6cents de la prise en charge biologique de l'infertilit4 masculine ont surtout port4 sur les m4thodes d'assistance m4dicale ~ la procr4ation. Parall61ement les techniques chirurgicales se sont am41io-r6es, en particulier avec rapport de la microchirurgie. Le traitement du varicoc61e repr6sente la situation oh la place de la chirurgie est la moins claire. Elle ne permet pas en particulier d'obtenir une augmentation du nombre de grossesses, malgr6 une am41ioration fr~quente des param6tres du sperme. Le traitement chirurgical de la cryptorchidie ne se discute plus apr6s 4chec d'un traitement hormonal. Le traitement de l'azoospermie excr4-toire repose sur des anastomoses microchirurgicales n6cessitant une 4quipe entraln6e. Ses meilleurs r~sul-tats sont obtenus dans les obstructions cong4nitales et ne doivent pas ~tre appr4ci4s avant 1 an postop6ratoire. De m~me la reperm6abilisation d~f4-rentielle permet d'obtenir 50% des grossesses en cas de technique microchirurgicale parfaite. La d6gradation des r4sultats avec le temps, impose une raise de sperme en banque de fa~on syst6matique avant la r6alisation d'une vasectomie. Les obstructions des canaux 4jaculateurs peuvent n6cessi-ter le recours ~ des gestes endoscopiques. Enfin les microponctions 6pidi-dymaires ont permis de r6aliser quelques f4condations. Mais les succ~s de I'ICSI ~ partir de pr61~vements chirurgicaux 4pididymaires ou d4f~ren-tiels conduisent ~ 41argir et ~ revoir consid4rablement les indications et les protocoles op6ratoires. Les indications sont ~ pr6ciser, et l'analyse gdn4tique est indispensable. Mais la qualit~ des conceptus obtenus reste la grande interrogation des ann4es ~ venir.
