High Levels of Antibodies Against Merkel Cell Polyomavirus Identify a Subset of Patients With Merkel Cell Carcinoma With Better Clinical Outcome

Abstract: High titers of MCV antibodies in a much higher proportion of patients with MCC than in controls confirmed the association between MCV infection and MCC. The findings also indicated that a better progression-free survival occurred in patients with high MCV antibody titers and suggested that there are at least two distinct etiologic causes of MCC.

“…The mean viral load has been reported to be 1-5 copies per cell, in agreement with integration of the viral genome into chromosomal DNA (1,90,93,94). Anti-VP1 antibodies have been detected in 88-100% of MCC cases (7,8,14,25). In addition, the capsid antibody titer in many of these MCC cases is much higher than observed in control populations.…”

“…In addition, the capsid antibody titer in many of these MCC cases is much higher than observed in control populations. The anti-VP1 geometric mean titer in MCC patients is 14-59 times higher than in controls (7,8). One explanation is that loss of immune surveillance in some patients enhances active replication of the virus and, as a consequence, the recombination of virus genome fragments with the host genome.…”

“…Other variables, exemplified by the growing literature regarding associations between the Merkel cell polyoma virus and outcomes in MCC, should be included. 11 Whenever possible, tissue banking of fresh tumor specimens is to be strongly supported. With regard to treatment, we continue to advocate offering SLNB to all patients with clinically localized MCC, as the implications of nodal basin microstaging are as yet not completely understood in this nodotrophic tumor.…”
