Hemostatic Alterations in Liver Disease: A Review on Pathophysiology, Clinical Consequences, and Treatment

Abstract: In most patients with acute or chronic liver failure, extensive changes in all pathways contributing to hemostasis are found. These hemostatic alterations concern both pro- and antihemostatic pathways, and therefore the net result of the hemostatic dysbalance is unclear. Although it is generally believed that patients with liver disease have a hemostasis-related bleeding tendency, this concept is challenged in recent literature. Although the bleeding problems in patients with liver disease are obvious, the cli… Show more

“…Surgeons were blinded for the allocated groups. The CVP at the beginning of the parenchymal transsection was significantly lower in the group with blood withdrawal (median 5 [range [2][3][4][5][6][7][8][9] 40-660] mL, P 5 .034). However, the two showed no statistical difference in postoperative outcomes.…”

“…Coagulopathy is an important hallmark of hepatic dysfunction [59,60]. Patients with ACLF have complex hemostatic defects leading to a delicate, unstable balance between bleeding and thrombosis [61].…”

“…7,23,24 The hemostatic system may be abnormal in patients who have compromised liver function and this may contribute to excessive bleeding during liver surgery. In particular, primary hyperfibrinolysis may occur as a result of the systemic inflammatory effect of surgery or due to hepatic injury that occurs during liver surgery.…”
