Has the in situ right internal thoracic artery been overlooked? An angiographic study of the radial artery, internal thoracic arteries and saphenous vein graft patencies in symptomatic patients☆

Abstract: Consideration should be given to the routine use of both internal thoracic arteries for coronary artery bypass grafting. When additional grafts are required, there is no evidence to suggest that either the radial artery or saphenous vein is superior.

“…Some retrospective studies suggested better immediate results using the RA as the second arterial graft [14,15] but other studies demonstrate the better patency of the RITA compared to the RA [16,17]. However we are far from reaching a consensus on this issue.…”

“…The use of the RA as a graft for coronary revascularization was already introduced in the 1970s, but shortly thereafter it was abandoned due to high incidence of vasospasm and comparatively poorer short-term and long-term patency rates than IMA [27,[122][123][124]. This was partly due to the inability to recognize RA spasm, but it was also due to lack of proper pharmacological tools to prevent this.…”

“…51 The SV graft still has a role as a graft to the moderately stenotic right coronary territory because of the high incidence of flow competition of composite RA grafts to the right coronary branches, 11,43,44,47,51,52 and the poor early graft patency of the RITA to the right coronary artery. 53,54 The advantage and disadvantage of the grafts are listed in Table 1.…”
