Although cervical cancer is preventable with early detection, it remains the second most common malignancy among women. An understanding of how proteins change in their expression during a particular diseased state such as cervical cancer will contribute to an understanding of how the disease develops and progresses. Potentially, it may also lead to the ability to predict the occurrence of the disease. With this in mind, we aimed to identify differentially expressed proteins in the plasma of cervical cancer patients. Plasma from control, cervical intraepithelial neoplasia (CIN) grade 3 and squamous cell carcinoma (SCC) stage IV subjects was resolved by two-dimensional gel electrophoresis and the resulting proteome profiles compared. Differentially expressed protein spots were then identified by mass spectrometry. Eighteen proteins were found to be differentially expressed in the plasma of CIN 3 and SCC stage IV samples when compared with that of controls. Competitive ELISA further validated the expression of cytokeratin 19 and tetranectin. Functional analyses of these differentially expressed proteins will provide further insight into their potential role(s) in cervical cancer-specific monitoring and therapeutics.

Endometriose ist eine chronische, rezidivierende Erkrankung. Da das Intervall zwischen den ersten klinischen Symptomen und der Diagnosestellung "Endometriose" immer noch durchschnittlich 6-9 Jahre beträgt [15], wäre es wünschens-wert, wenn betroffene Frauen durch nichtinvasive Untersuchungsmethoden frühzeitig erkannt und rechtzeitig behandelt würden. Je früher die Behandlung beginnt, desto besser ist das therapeutische Ergebnis und desto seltener kommen Rezidive vor [24].Unsere Übersichtsarbeit hat zum Ziel, neue molekularbiologische Marker vorzustellen, die in Zukunft zur Diagnosestellung der Endometriose herangezogen werden könnten. Bisherige DiagnostikDie gesicherte Diagnose einer Endometriose wird bisher ausschließlich durch einen invasiven Eingriff mit einer anschließen-den histologischen Untersuchung gestellt. Chirurgische Interventionen stellen trotz der minimal invasiven Weise eine Belastung für die Patientinnen dar, sind mit gewissen operativen Risiken behaftet, erfordern normalerweise eine Vollnarkose und sind kostenintensiv.Bei der Laparoskopie werden verdäch-tige Herde entnommen und histologisch untersucht. Die Sensitivität dieser Methode beträgt 97 und die Spezifität 77 [27]. Der histologische Befund bestätigte eine vermutete Endometriose nur in 60-85 der Fälle [7]. Laut den "Guidelines for the diagnosis and treatment of endometriosis" der ESHRE SIG Endometriosis (European Society of Human Reproduction and Embryology Special Interest Group Endometriosis) darf aber mit Ausnahme von Ovarialendometriomen Endometriose auch dann laparoskopisch diagnostiziert werden, wenn die Histologie dies nicht bestätigen kann [18]. Die medizinische Behandlung einer Peritonealendometriose wird als genauso effektiv betrachtet wie eine chirurgische Therapie [7], dagegen sind Ovarialendometriome und die Endometriose des Septum rectovaginale ausschließlich operativ erfolgreich anzugehen. Bei einer Infertilität profitieren betroffene Patientinnen mit minimaler oder milder Endometriose von einer operativen Entfernung der Endometrioseareale. Das Endometriosestadium korreliert mit dem Grad der Infertilität. Die Schwangerschaftsraten erhö-hen sich bei der operativen Laparoskopie (30,7) im Vergleich zur alleinigen diagnostischen Laparoskopie (17,7; [21]). E Für den weiteren Krankheitsverlauf ist es wichtig, die Endometriose so früh wie möglich zu diagnostizieren. Bei Endometrioseverdacht werden die transvaginale Sonographie, die endorektale Sonographie und manchmal auch die kostenintensivere Magnetresonanztomographie (MRT) eingesetzt [3, 23]. Die transvaginale Sonographie ist hervorragend zur Diagnose eines Ovarialendometrioms und die transrektale Sonographie zur Diagnostik einer rektalen Infiltration geeignet. Mithilfe der MRT kann z. B. sehr gut eine tief infiltrierende Endometriose im Bereich des rektovaginalen Septums, aber auch eine Adenomyosis uteri diagnostiziert werden. Dagegen sind minimale peritoneale Herde mit keiner der oben erwähnten Techniken darstellbar.
