Haematoma in a hydrocele of the canal of Nuck mimicking a Richter’s hernia

Abstract: We report a haematoma in a hydrocele of the canal of Nuck in a 69-year-old female. She presented with a right-sided groin swelling, the differential for which included an irreducible inguinal hernia or haematoma given her aspirin and clopidegrel use. Successful treatment involved evacuation of the haematoma with excision of the sac. Despite a high index of suspicion for a haematoma, these swellings should ideally be explored given the potential for co-existence of a hernia.

“…We first considered endometriosis, hydrocele of the canal of Nuck, nerve involvement, and tissue injury due to the ligature as the cause of pain. Although endometriosis and hydrocele of the canal of Nuck have been reported to cause pain, these were negative on pathological examination . There was also no clear finding of endometriosis in the hernia during the initial surgery.…”

“…Surgical management of a cyst of the Canal of Nuck includes open or laparoscopic excision of the cystic structure with concomitant closure of the inguinal internal defect primarily with the use of a mesh (24,31,32). The appropriate surgical approach is tailored based on the extent of the disease, the accuracy of preoperative diagnosis and the co-existence of an inguinal hernia.…”

“…Treatments for a hydrocele of the canal of Nuck include surgical excision of the cyst with closure of the inguinal internal ring defect [ 9 ] and simultaneous inguinal hernia repair if required [ 15 ]. Surgical excision is effective as it is both therapeutic and diagnostic [ 16 ].…”
