Introducción: la detección de enfermedad arterial periférica permite identificar a un grupo de alto riesgo cardiovascular. La prueba del índice tobillo-brazo con esfigmomanómetro digital es una técnica muy fiable, fácil de aplicar y de interpretar. Objetivos: determinar la frecuencia y características clínicas de la enfermedad arterial periférica en sujetos adultos de Atención Primaria de Areguá, Paraguay, en el 2019. Metodología: estudio observacional, descriptivo, prospectivo, multicéntrico. Se incluyó a varones y mujeres mayores de 50 años que asisten a tres Unidades de Salud Familiar de Areguá, Paraguay, en el 2019. Previo consentimiento informado, todos fueron sometidos a determinaciones socioeconómicas, clínicas y detección del índice tobillo-brazo con esfigmomanómetro digital. Se consideró anormal todo índice tobillo-brazo <0,9. Resultados: se incluyeron 124 pacientes, con edad media 64±8 años. Predominaron los sujetos del sexo femenino, con escolaridad primaria, casados y sin independencia económica. Las comorbilidades más frecuentes fueron la hipertensión arterial y diabetes mellitus. Se detectaron 7 casos (5,6%) compatibles con arteriopatía periférica. Entre éstos, sólo 43% referían claudicación intermitente de miembros. Conclusiones: la frecuencia de índice tobillo-brazo compatible con arteriopatía periférica fue 5,6%. Entre éstos, los síntomas de claudicación de miembros fueron referidos por 43%.

This study aimed to evaluate the effect of a multimodal supervised exercise training (SET) program on walking performance for 12 months in patients with symptomatic lower extremity peripheral artery disease (PAD). Consecutive patients with Fontaine stage II PAD participating in the SET program of our hospital were retrospectively investigated. Walking performance, assessed using a treadmill with measures of the pain-free and maximal walking distance (PFWD, MWD, respectively), and 6 min walking distance (6MWD), were tested before and following SET, as well as at 6 and 12 months after SET completion. Ninety-three symptomatic patients with PAD (65.0 ± 1.1 y) were included in the study. Following SET, the walking performance significantly improved (PFWD: +145%, p ≤ 0.001; MWD: +97%, p ≤ 0.001; 6MWD: +15%, p ≤ 0.001). At 6 months, PFWD (+257%, p ≤ 0.001), MWD (+132%, p ≤ 0.001), and 6MWD (+11%, p ≤ 0.001) remained significantly improved compared with the pre-SET condition. At 12 months, PFWD (+272%, p ≤ 0.001), MWD (+130%, p ≤ 0.001), and 6MWD (+11%, p ≤ 0.001) remained significantly improved compared with the pre-training condition. The walking performance remained significantly improved in both women and men for up to 12 months (p ≤ 0.001). Multimodal SET is effective at improving walking performance in symptomatic patients with PAD, with improvements lasting up to 12 months.

With the changing disease spectrum, atherosclerosis has become increasingly prevalent worldwide and the associated diseases have emerged as the leading cause of death. Due to their fascinating physical, chemical, and biological characteristics, nanomaterials are regarded as a promising tool to tackle enormous challenges in medicine. The emerging discipline of nanomedicine has filled a huge application gap in the atherosclerotic field, ushering a new generation of diagnosis and treatment strategies. Herein, based on the essential pathogenic contributors of atherogenesis, as well as the distinct composition/structural characteristics, synthesis strategies, and surface design of nanoplatforms, the three major application branches (nanodiagnosis, nanotherapy, and nanotheranostic) of nanomedicine in atherosclerosis are elaborated. Then, state‐of‐art studies containing a sequence of representative and significant achievements are summarized in detail with an emphasis on the intrinsic interaction/relationship between nanomedicines and atherosclerosis. Particularly, attention is paid to the biosafety of nanomedicines, which aims to pave the way for future clinical translation of this burgeoning field. Finally, this comprehensive review is concluded by proposing unresolved key scientific issues and sharing the vision and expectation for the future, fully elucidating the closed loop from atherogenesis to the application paradigm of nanomedicines for advancing the early achievement of clinical applications.
