Grading of Aortic Valve Stenosis at 64-Slice Spiral Computed Tomography
Abstract: Planimetric AVA measurements on MSCT allows for an accurate grading of aortic valve stenosis severity. However, AVA measurements on MSCT are usually larger than measurements on TTE and CATH. Consequently, the thresholds for discriminating between different severity grades have to be adjusted in MSCT.

“…According to the literature the MSCT-determined AVA strongly correlates with cardiac ultrasound findings and the modality shows excellent sensitivity and specificity to detect severe stenosis [2–6, 10–13]. Messika-Zeitoun et al [8] presented results comparable to our findings.…”

“…Lembcke et al conducted a study with 160 patients using 64-MDCT and TTE. They found 0.17 ± 0.24 cm 2 mean difference between MDCT and TTE measurements [ 32 ]. In our study we observed 0.19 ± 0.27 cm 2 and 0.10 ± 0.18 cm 2 mean differences in the comparisons of TTE with manual and semiautomatic MDCT measurements, respectively.…”

“…A comprehensive evaluation of the AV with the possibility of left ventricular function and coronary artery anatomy assessment has been validated in patients with aortic stenosis (AS) with high concordance for planimetry evaluation compared to transesophageal echocardiogram (TEE), high sensitivity, and specificity for concomitant coronary assessment 5–8 …”

“…A comprehensive evaluation of the AV with the possibility of left ventricular function and coronary artery anatomy assessment has been validated in patients with aortic stenosis (AS) with high concordance for planimetry evaluation compared to transesophageal echocardiogram (TEE), high sensitivity, and specificity for concomitant coronary assessment. [5][6][7][8] MDCT is considered an essential tool to plan for transcatheter AV replacement (TAVR), permitting accurate sizing of the complex three-dimensional, noncircular geometry of the aortic root, detection of anatomic features leading to potential risk complication, and of course, the definition of best vascular access. [9][10][11][12][13][14][15] In patients post TAVR, MDCT is not routinely recommended.…”
