Gracilis Muscle Transposition for Fistulas Between the Rectum and Urethra or Vagina

Abstract: Gracilis muscle transposition is a viable option for repairing fistulas between the urethra, vagina, and the rectum, especially after failed perineal or transanal repairs. It is associated with low morbidity and a good success rate. Underlying Crohn's disease and previous radiation are associated with poor prognosis.

“…Successful use of the gracilis muscle interposition has been reported for Crohn's RVF repairs, especially after other failed repairs. Zmora et al [38] reported on their use of the gracilis flap in nine patients for various causes of fistula, including 3 rectourethral fistulae, 1 pouchvaginal fistula, and 5 RVF (2 CD associated RVF). All patients underwent fecal diversion.…”

“…Other studies have not demonstrated any advantage of a diverting stoma [6,[14][15][16]. Some have suggested that the interposition of a gracilis flap may avoid the need for a permanent ileostomy in patients with recurrent pouchvaginal fistulae [17,18]. In our series, temporary diversion was used in all patients and it may have contributed to the high healing rate even in some recurrent fistulae.…”

“…Zmora et al noted that three of nine patients with graciloplasty for rectovaginal and urethral fistulas had postoperative minor wound sepsis with no other short or long-term complications. 10 In a review of the literature since 1980, Penninckx et al 5 compared healing rates after various surgical procedures for RVF. They concluded that all types of repair seemed to be equally effective.…”

“…[5][6][7]10,17 In these series (including an overall rate of only 25%), the overall healing rate after GMT was 84% (21/25) after a mean follow-up of 2 years. The present study, with a median follow-up of 28 months after stoma closure, compares favorably with a success rate of 88%.…”
