Glue Ear in Children with Cleft Lip and Palate: An Update

Abstract: Cleft lip/palate (CLP) in children is psychologically stressful for family members and debilitating for the patients themselves. These children must undergo a series of major invasive surgeries, including cleft lip repair surgery, cleft palate repair surgery, bone grafting surgery, and/or dental implant surgery. Unfortunately, the clinical significance of otitis media with effusion (OME), a complication associated with CLP, is often overlooked and very few studies have explored this condition in depth. In this… Show more

“…Studies have been able to establish a link between the severity of the cleft, type of ventilation tube placed, placement of ventilation tubes during the palatoplasty and the incidence of middle ear problems. As the male gender is linked to both a higher incidence of otitis media and to more severe forms of palatal clefting, it has also been described as a signifi cant factor [4,[28][29][30]. The results of our study, however, could not confi rm these previous fi ndings, but could only point out that a higher age was an independent risk factor for middle ear complications.…”

“…70 OME is known to have associated problems such as ossicular necrosis, tympanosclerosis, retraction pockets with cholesteatoma, atrophy of tympanic membrane and atelectasis as its possible sequelae. [71][72][73][74] Studies have recommended routine follow-up and audiologic assessments for patients with cleft palate for timely diagnosis and intervention of OMEs. [71][72][73][74] Timely evaluation and intervention can possibly avert or at least reduce associated problems and their possible effects on quality of life in the cleft population.…”

“…Children born with CLP are faced with the physical and emotional challenges of undergoing several major operations early in life. Many of these children undergo ventilation tube insertion (VTI), also known as tympanostomy or grommet insertion, to mitigate eustachian tube dysfunction and resultant fluid accumulation within the middle ear (Kuo, 2018). If left untreated, this fluid can potentially cause severe otitis media with effusion (OME) and ensuing hearing loss.…”

“…OME occurs in at least 90% of patients with CLP in comparison with 5%–30% of patients without CLP (Stool and Randall, 1967; Daly et al, 1999). Sequelae of OME include hearing and speech impairments, which can have severe implications on a child's development, learning ability, social interactions, and overall mental and physical wellbeing (Kuo, 2018). Furthermore, OME and the ensuing complications greatly increase the burden of cleft care, both for the patients and healthcare system alike (Stool and Randall, 1967; Grant et al, 1988).…”
