Global Distribution of Infectious Diseases Requiring Intensive Care

Abstract: This article describes infectious diseases that are of special importance to intensivists. The emphasis on epidemiology notwithstanding, it also addresses clinical, diagnostic, and treatment issues related to each infection described. The discussion avoids terrorism-related aspects of these infections, because they were very well covered in the October 2005 issue of the Critical Care Clinics.

“…The global importance of this disease is highlighted by estimates that leptospirosis causes severe complications for 1 million people annually. [5][6][7] Immunity to Leptospira in humans is mediated primarily by antibodies directed towards lipopolysaccharide. 8 This immunity is serovar specific and has led to a search for conserved cell surface proteins that might elicit cross-serovar immunity.…”

“…In the last decade, endemic and epidemic severe pulmonary hemorrhage has increasingly become recognized as an important manifestation of leptospirosis. The disease has a worldwide distribution but is more common in the tropics where conditions for its transmission are particularly favorable [2, 4]. In Brazil, the disease is endemic, with outbreaks during the rainy seasons, when flooding occurs [5, 6].…”

“…Malaria leads to another 1.5 to 3.5 million deaths annually [ 21 ]. The global incidence of tetanus is approximately 1 million cases per year, with a case fatality rate that can approach 50% [ 22 ]. Tetanus is a particularly frustrating critical illness given that it can be prevented by a simple and safe series of childhood immunizations.…”
