Glandular odontogenic cyst: a rare entity revealed and a review of the literature

Abstract: A glandular odontogenic cyst (GOC) is a developmental cyst that is a clinically rare and histopathologically unusual type of odontogenic cyst. GOCs are now relatively well-known entities; their importance relates to the fact that they exhibit a propensity for recurrence rates from 21% to 55%, similar to odontogenic keratocysts, and may be confused microscopically with central mucoepidermoid carcinoma. Furthermore, some microscopic features of GOCs may also be found in dentigerous, botryoid, radicular and surgi… Show more

“…4,14,16,[20][21][22][23] Analysis of the expression of epithelial membrane antigen and mammary serine protease inhibitor (MASPIN) in doubtful cases of GOC can be helpful to distinguish between GOC and central MEC, demonstrating a minimal or a lack of expression in GOC cases. 16,18 Special stains, such as Periodic acid-Schiff, Mucicarmine and Alcian blue, have been used to identify and distinguish between clear or vacuolated cells and mucous cells. 5,14 On the other hand, immunohistochemical analysis could be an adjunct in the understanding of GOC behaviour.…”

“…In fact, the presence of prosoplasia in the cystic lining with mucous cells and ciliated cells highly mimics microscopic features of GOC 5 . The histopathological features of a low‐grade central MEC can overlap with the features of GOC 5,16–18 . Besides the microscopic similarities, central MEC also affects middle‐aged individuals and, when located within the mandible, it may cause bone expansion and produce a radiographic appearance of a well‐defined unilocular or multilocular radiolucency 19 …”

“…Some surgeons prefer marginal or segmental resection due to the tendency of GOC to recur after conservative treatment. [ 8 9 10 ] The rate of recurrence has been found to vary with its radiographic complexity. Patients with large multilocular lesions and cortical perforation show tendency for recurrence.…”
