Abstract: This study clearly demonstrated that MCT activates ghrelin and increases NPY, suggesting that nutritional supplementation with MCT may be effective for the treatment of AN patients in an emaciated state.

“…It is to note that especially fat intake predicted ghrelin values in plasma of patients with AN [ 93 ], likely related to the need of fatty acids for ghrelin’s acylation in the stomach [ 24 ]. This is further supported by the finding that consumption of medium-chain triglycerides elevates ghrelin levels by activation of GOAT [ 94 ].…”

“…Medium‐chain triglycerides (MCTs) provide the liver with an energy source, thereby reducing fat deposition as adipose tissue by bypassing chylomicron formation to lymphatic transport. A diet high in MCT may affect the transformation of inactive to active (acetylated) ghrelin (Kawai et al, ). In PWS, ghrelin is elevated throughout the nutritional phases, and more recent studies suggest a role for the active form in disease pathogenesis (Beauloye et al, ; Kweh et al, ).…”

“…Appetite measurement by visual analog scale has not been validated in AN, and due to lack of a randomized, placebo-controlled group and blinding of the investigators, conclusions from that study cannot be drawn but should inspire subsequent studies. In another study of 30 patients with severe AN, medium-chain triglyceride (MCT) supplementation increased the level of acylated (active) ghrelin levels in a dose-dependent manner (154). However, the increase in body weight did not differ significantly between low, moderate and high MCT supplementation (154).…”
