Gestational diabetes: An overview with attention for developing countries

Abstract: Gestational diabetes mellitus (GDM) is defi ned as a glucose intolerance that occurs for the fi rst time or it is fi rst identifi ed during pregnancy. Th e GDM etiology is multifactorial. It has not completely been established yet and several known risk factors may contribute to its onset. To date, there are no shared guidelines on the management and follow-up, especially regarding the lowincome countries. In this paper, we describe the state of art about epidemiology, physiopathology, diagnosis, and managemen… Show more

“…Gestational diabetes mellitus (GDM) is defined as any degree of carbohydrate intolerance, with onset or first recognition during second or third trimester of pregnancy ( 1 ). The prevalence in the world is increasing, and approximately 7% of all pregnancies are complicated by GDM ( 1 ), with major risk factors identified in previous history of GDM, history of macrosomia, familiarity for type 2 diabetes (T2D), elevated maternal age, and pre-pregnancy obesity ( 2 , 3 ).…”

“…GDM is a pregnancy complication affecting both the mother and the baby even after delivery. Adverse complications for the mother such as preeclampsia, still birth, abortion, early delivery, caesarean delivery, risk for cardiovascular diseases and T2DM and others, and for the baby such as macrosomia, fetal malformation, obesity, risk for T2DM, and among others have been noted [11,14,15]. And to prevent these complications, a good diagnostic criterion that will screen the mother early during pregnancy is needed.…”

“…Current test done to diagnose GDM is oral glucose tolerance test (OGTT) and results are commonly interpreted using the criteria set by either the World Health Organization (WHO), American Diabetes Association (ADA), or International Association of the Diabetes and Pregnancy Study Groups (IADPSG) [6,10,11]. Three criteria are available for the diagnosis of GDM and selection of criterion is dependent on the laboratory protocol or on the preference of the obstetrician-gynecologist.…”

“…GDM is the most common pregnancy metabolic disorder with an increasing prevalence ranging from less than 1 to 28% [16][17][18] that parallels the worldwide epidemic of T2DM [19]. The frequency of occurrence depends on diagnostic methods, ethnicity, and body composition [20]. Some ethnic groups have been long associated with an increased risk of GDM, and the prevalence seems particularly higher among women from South Asia and South East Asia than from Caucasian, African-American, and Hispanic communities [21].…”
