General health maintenance in IBD

Abstract: Patients with inflammatory bowel disease (IBD) often rely on their gastroenterologist for healthcare maintenance. In addition, the gastroenterologist also provides guidance to the patient's primary care physician on a broad range of issues such as vaccinations, osteoporosis screening, and cancer/dysplasia surveillance. Appropriate vaccinations should be administered to patients with IBD, particularly those likely to receive immunosuppression. Live virus vaccines are not appropriate for patients on immunosuppre… Show more

“…Members of the gastroenterology team are oft en the only clinicians that a patient with IBD will see. As such, it is incumbent upon gastroenterologists to take a proactive role in the health care needs of their IBD patients (3)(4)(5). It is crucial to clarify with the patient the limits of the specialist's responsibilities and delegate routine health care issues to the primary care clinician.…”

“…However, this hypothesis merits exploration in future studies. Expert groups and guidelines recommend close surveillance of bone density in the IBD cohort with both primary and secondary prevention of osteoporosis through calcium and vitamin D supplementation, encouraging physical activity, optimal control of disease activity, judicious use of steroids, and bisphosphonate or other medical therapy when indicated [1,4,5,27].…”

“…Maintenance of bone health is an important goal in patients with inflammatory bowel disease [IBD; ulcerative colitis (UC), Crohn's disease (CD)] [1]. High prevalence of reduced bone mineral density (BMD) has been reported from several IBD cohorts, with some studies reporting prevalence as high as 54% [2][3][4][5][6][7][8].…”

“…Certain health care maintenance issues, as well as the overall care for the patient, become important when considering initiation of IS therapy [9]. The definition of "immunosuppressed" in this context is the use of corticosteroids at doses greater than 20 mg for longer than 2 weeks, effective doses of immunomodulators, methotrexate, or biologics, and significant protein-calorie malnutrition.…”
