Abstract: An increasing number of women have been diagnosed with spondyloarthritis (SpA) in recent decades. While a few studies have analyzed gender as a prognostic factor of the disease, no studies have addressed this matter with a large number of patients in South America, which is a peculiar region due to its genetic heterogeneity. The aim of the present study was to analyze the influence of gender on disease patterns in a large cohort of Brazilian patients with SpA. A prospective study was carried out involving 1,50… Show more

“…On the other hand, women referred higher involvement of peripheral joints, dactylitis and enthesitis, associated with higher disease activity (BASDAI) and worse quality of life (ASQoL). Similar data were observed in 1505 patients from the Brazilian Registry of Spondyloarthritis (RBE) [26] and 1514 patients from the Spanish SpA Registry (REGISPONSER), [27] as well as in a British study [28] and in the French DESIR cohort, which included patients with axial SpA of recent onset. “…The male:female ratio was 2.6:1, which is significantly less than previously reported 7:1 ratio in juvenile AS [13]. This ratio is similar to recent studies in adult spondyloarthropathy (SpA) where the male: female ratio ranged from 2 to 5:1 [14-16]. HLA-B27 was present in 40.6% of the subjects with MRI (+) sacroiliitis, similar to a previous report by Pagnini et al in 17 children with enthesitis-related arthritis (ERA) and sacroiliitis [17], but much less than reported in adult AS where HLA-B27 was present in >90% [18].…”

“…Estrogen is involved in a variety of immunity‐related processes, such as T cell differentiation, cytokine production, and dendritic cell regulation . A Brazilian cohort study showed that the prevalence of HLA‐B27 is higher in male patients with SpA than female patients . We hypothesize that sex hormone, the X‐chromosome, or other genetic factors may be partially involved in the pathogenesis of SpA.…”
