Introdução: A mamoplastia de aumento é um procedimento cirúrgico amplamente realizado globalmente, visando primariamente à melhoria da estética mamária e da autoconfiança. Apesar dos benefícios estéticos, podem surgir complicações pós-operatórias, algumas subdocumentadas ou carentes de discussão aprofundada na literatura científica. Uma dessas complicações notáveis é a galactorreia, caracterizada pela secreção involuntária de leite pelas glândulas mamárias. Objetivo: Este artigo visa investigar e analisar as evidências disponíveis relacionadas à associação entre mamoplastia de aumento e o surgimento de galactorreia pós-operatória. Métodos: Foi realizada uma revisão bibliográfica nas bases de dados PubMed e SciELO, abrangendo os últimos cinco anos, com alguns artigos mais antigos pela relevância do tema. Diretrizes de associações brasileiras também foram consultadas. Resultados: A galactorreia após mamoplastia de aumento é uma ocorrência rara, com incidência variando entre 0,1% a 0,5% nos estudos revisados. Fatores de risco incluem histórico de disfunções hormonais, uso de determinados medicamentos, e estimulação excessiva dos seios durante a cirurgia. A prolactina é o principal hormônio implicado na galactorreia, com intervenções cirúrgicas que interferem na glândula pituitária ou no tecido mamário podendo elevar seus níveis. Conclusões: Esta revisão destaca a importância do reconhecimento precoce e manejo adequado da galactorreia pós-mamoplastia, estimulando uma avaliação minuciosa dos pacientes e uma abordagem informada para o diagnóstico e tratamento desta condição.
