Abstract: ELIOT is a safe method for treating conservatively operated breasts, avoids the long period of postoperative radiotherapy, and reduces drastically the cost of radiotherapy. ELIOT reduces radiation to normal tissues and organs. Results on short-term and middle-term toxicity up to 5 years of follow-up are good. Data on local control are encouraging.

“…The idea of a single shot treatment, as proposed by the Milan group, is tempting especially in low risk patients [12,24]. Major criticisms point out that the investigated dose of 21 Gy is outside the tested dose ranges for equal tumor effects in the linear quadratic model.…”

“…In this context, one of the subprocesses during breast cancer IOERT treatment is the protection of internal normal tissue from radiation leakage 6 , 7 . It involves the surgeon positioning a metal disc between the deep face of the patient's residual breast and the pectoral muscle, and suturing the resected mammary gland.…”

“…In high‐dose IOERT, the use of a shielding disc is mandatory (6) . Tumor tissue must be irradiated uniformly, almost to within 90% of the maximum prescribed dose (23Gy).…”

“…Electron intra-operative radiotherapy technique has been described in previous papers [6][7][8][9]. Briefly, two mobile linear accelerators ( Figure 1) were used to deliver electrons, a Novac7 (Hitesys Srl, Latina, Italy) and a Liac (Info&Tech, Roma, Italy), installed in two different operating rooms.…”

“…With the new technique, which we defined electron intra-operative therapy (ELIOT), the mobile linear accelerator delivers a single dose of radiation with electrons to the involved quadrant of the breast during surgery, shortening the radiotherapy course from six weeks to one single session during surgery [6][7][8][9].…”
