Frequency of Hashimoto’s Thyroiditis Antecedents in the History of Children and Adolescents with Graves’ Disease

Abstract: Background: The development of Graves’ disease (GD) from Hashimoto’s thyroiditis (HT) has sporadically been reported, but no data are available concerning the prevalence of this sequence of events in GD patients. Our aim was to ascertain HT antecedents in the history of GD children in order to assess for the first time the relative frequency of the event sequence leading from HT to GD in a pediatric population. Study Population and Results: In 105/109 patients, no HT antecedents were documented at GD presentat… Show more

“…This is not surprising considering that such a conversion is known to be detectable in the natural history of patients with HT (42,43,44,45,46) and has also been specifically described in children with either TS (29,30,31) or DS (38,47). Moreover, the shifting HT/GD has been, just recently, found to be more common in GD children with these chromosomal aberrations than in those without them (48).…”

“…The close link between autoimmune thyroiditis and Graves’ disease is further demonstrated by the development of autoimmune hypothyroidism in up to 15% of patients with Graves’ disease, 10–15 years after the index illness,5 and the more infrequent transition of autoimmune thyroiditis into Graves’ disease 6…”

“…This is not surprising considering that an evolution from HT to GD has occasionally been observed in the natural history of children with HT. 31 Moreover, such a conversion has just recently been demonstrated to occur even more often in patients with DS than in those without DS. 32 To sum up, many studies on the relationships between DS and thyroid function include heterogeneous groups of patients with both autoimmune and idiopathic thyroid disorders, whereas in this study only selected groups with autoimmunity were recruited, thus making interpretation of results more transparent.…”
