Influence of Methodological Variables on Fracture Strength Tests Results of Premolars with Different Number of Residual Walls. A Systematic Review with Meta-Analysis

The aim of the current meta-analysis was to assess the impact of methodological variables in performing fracture strength tests of upper premolars. Medline (Pubmed), Embase and Google Scholar were screened for studies performing ex vivo fracture strength tests of intact upper premolars or premolars with 0, 1 or 2 walls lost. The outcome variable for each study was the maximum breaking load expressed in Newton (kg × m/s2). Methodological variables (i.e., simulation of the periodontal ligament, load inclination, tip position, tip diameter and thermocycling) were registered to perform subgroup analyses and meta-regression. Overall, 25 studies and 78 study groups were included in the meta-analysis. Intact premolars (17 study groups) were not significantly influenced by any of the methodological variables considered. Subgroup analysis for load inclination (30°/45° vs. 90°/150°) was significant for premolars with 0 (10 study groups), 1 (6 study groups) and 2 (45 study groups) walls lost; thermocycling was significant for premolars with 1 and 2 walls lost. A strong methodological heterogeneity across studies evaluating the fracture strength of upper premolars was highlighted, especially when 0, 1 or 2 walls were lost. Further studies are needed to standardize the methodology used in order to allow for across-studies comparisons.

Aim: In this study was evaluated the fracture resistance of endodontically treated maxillary premolars restored with different restorative materials. Methods: Sixty maxillary premolars were submitted to the same mesio-occlusal-distal cavity preparation, endodontic treatment and divided into 5 groups (n = 10): Coltosol Group – GCO restored with calcium silicate material; Glass Ionomer Cement Group – GGIC, restored with Maxxion R; Modified Glass Ionomer Cement – GMGIC, restored with Gold Label 2; Composite Group - GC, restored with Z100, and the positive control group (GP) - left unrestored. One group remained intact (n=10) serving as negative control (GN). Samples were subjected to fracture resistance testing by the universal testing machine until fracture occurred and was registered in newtons (N). Fracture pattern was assessed and described as favorable or unfavorable. The results were statistically analyzed by 1-way analysis of variance and the post hoc Tukey test with significant statistical difference at P < 0.05. Results: Higher fracture resistance results were found for GC (1,128.35 ± 249.17), GMGIC (1,250.77 ± 173.29), and GN (1,277.22 ± 433.44) (P < .05). More favorable fractures were observed in the GCO (6), GC (7), and GN (7) (P < .05). Conclusion: Teeth restored with composite and modified GIC presented the same resistance as intact teeth. Teeth restored with Coltosol and GGIC presented similar resistance to unrestored teeth.

Background: A myriad of materials and procedures have been recommended for restoring the root-filled teeth with significant loss of tooth structure and the most common method being the use of “posts.” Material and Methods: A mesio-occluso-distal preparation was done on eighty intact maxillary first premolars, followed by access preparation and cleaning and shaping with rotary files. All samples were obturated and divided into four groups ( n = 20). In Group 1, 3-mm gutta-percha was removed and restored using composite resin; Group 2 – Group 1 + crowns; Group 3 – prefabricated fiber post + crowns; and Group 4 – prefabricated metal post + crowns. All the groups were subdivided into eight groups ( n = 10), fractured with 90° vertical and 45° oblique loading by a universal testing machine. Statistical Analysis: Fracture resistance was analyzed and compared by means of two-factorial two-way analysis of variance and Scheffe's post hoc test. Results: Under 90° loading, the fracture load of teeth restored with the Nayyar's composite resin core and crown was greatest. Under 45° loading, the fracture load of teeth restored with prefabricated metallic posts was significantly ( P < 0.05) less than that in other groups. Under 45° load, group with fiber post exhibited significantly ( P < 0.05) more number of favorable fractures than other groups. Conclusion: Under the conditions of vertical and oblique loadings, fiber post and composite resin core with a full-coverage crown is the most effective restorative modality for protecting the remaining tooth structure in pulpless teeth.
