Folic Acid Fortification of Wheat Flour: Chile

Abstract: Neural tube defects (open spina bifida, anencephaly, and encephalocele) represent the first congenital malformations to be preventable through public health measures such as supplementation and/or food fortification with folic acid. In Chile, starting in January 2000, the Chilean Ministry of Health legislated to add folic acid to wheat flour (2.2 mg/kg) to reduce the risk of NTDs. This policy resulted in an estimated mean additional supply of 427 microg/d in significant increases in serum folate and red cell f… Show more

“…However, women like the participants in our study comprise the group that fortification programs were designed to benefit. Our documentation of a large post- versus pre-fortification difference in folate status among OCPP participants adds importantly to the argument that a reduction in NTD occurrence post-fortification observed in Brazil [15] and other countries [11, 2325] was mediated by a dramatic increase in folate status and decline in folate deficiency among high-risk women. Improving folate status in women of child-bearing age may have other benefits.…”

“…Canada, South Africa, Costa Rica, Chile, Argentina, and Brazil also have reported declines in NTDs (19%–55%) since the initiation of folic acid food fortification [33,34,35,36,41,42,43,44,45,46]. The magnitude of the decrease in NTD prevalence observed in each country has been dependent on a number of factors.…”

“…In Chile, a wheat flour folic acid fortification program showed an increase in blood folate levels [13] and a decrease in the risk of NTDs [14,15]. An ex-post economic evaluation of the program should provide useful information for policy makers in other countries where congenital malformations are an important cause of infant mortality.…”
