Focal myositis associated with S‐1 radiculopathy: Report of two cases

Abstract: Two cases are described of pseudotumoral calf hypertrophy after laminectomy for a compressive S-1 radiculopathy. The serum creatine kinase (CK) level was normal or mildly elevated. T2-weighted magnetic resonance imaging (MRI) showed calf enlargement, with an increased signal of the medial head of the gastrocnemius muscle. Electromyography revealed fibrillation potentials and positive sharp waves, but no complex repetitive discharges in the affected gastrocnemius muscle, with motor unit potentials having mixed … Show more

“…Treatment is focused on rhabdomyolysis which require adequate hydration to reduce serum myoglobin and protection of renal function. In all of these conditions, counseling is essential to limit drug abuse (Streichenberger et al, 2004; Chawla et al, 2005; O’Connor and McMahon, 2008; Brancaccio et al, 2010; Kasaoka et al, 2010). …”

“…However, as an uncommon finding, particularly in the case of chronic partial denervation, an enlargement of the partially denervated muscles may occur 6. This may be caused by pseudohypertrophy with replacement of muscle fibers by fatty tissue, or by edema, focal myositis,8 or true hypertrophy induced by spontaneous repetitive motor unit discharges. Because there were no signal changes on unenhanced T1‐weighted images in our patient, we consider fatty replacement unlikely.…”
