[12][13][14] It involves creating a submucosal tunnel through an esophageal mucosal incision approximately 10 cm proximal to esophagogastric junction and then dissecting the circular muscle fibers beginning at approximately 3 cm distal to the mucosal entry site and extending 2 cm into the cardia.
