Abstract: OBJECTIVES: This study aims to examine the feasibility and clinical course after minimally invasive David procedure compared with those via a conventional median sternotomy. METHODS:One hundred and ninety-two consecutive patients who underwent elective valve-sparing aortic root replacement (David procedure) with or without additional cusp repair for aortic regurgitation (n = 17, 8.9%), dilatation of the aortic root (n = 95, 49.5%) or a combination of both pathologies (n = 80, 41.7%) were included. Patients wit… Show more

“…This prospective study quantified the level of pain, anxiety and depression, and QOL after ministernotomy aortic surgery. VAS pain, HADS and QOL scores recorded at baseline were comparable to those published in other studies 9–13 . On average, VAS pain scores were 6.5 out of 10 on POD 1 and decreased to 2.7 out of 10 at discharge.…”

“…QOL parameters were similar between both groups at 6 months. Wachter, et al also found that after ministernotomy valve‐sparing ARR, patients tended to have higher QOL measures (though not statistically significant) in all SF‐36 categories at 3 years when compared to their full‐sternotomy counterparts 12 . Walther, et al found improved pain and better QOL in a large group of patients who underwent minimally invasive surgery 9 .…”

“…This was confirmed by Mikus et al 1 in a study of 53 patients who received a Bentall procedure using PUS. Finally Wachter et al 19 proved the safety of valve‐sparing aortic root replacement with David procedure via PUS when he compared the two approaches—PUS (117 Patients vs FMS (75 Patients).…”

“…However, we were not able to address echocardiographic characteristic for long-term durability. [13][14][15] Two other aspects regarding possible difference in longterm outcome of the procedure have been discussed in the past. First, the influence of a bicuspid valve and second, the role of cusp repair.…”
