Aeromonas species are emerging human enteric pathogens. However, systematic analysis of Aeromonas species infection in human gastroenteritis in comparison with other enteric bacterial pathogens in the Australian population is lacking. Here we analysed the isolation of Aeromonas species and other bacterial pathogens in five consecutive years (2015–2019) from 375,842 stool samples of patients with gastroenteritis in a large Australian diagnostic laboratory and identified a subset (48 isolates) of Aeromonas isolates to species level, using multilocus phylogenetic analysis. Aeromonas species were the third most common bacterial pathogens, following Campylobacter and Salmonella species. Aeromonas infection rate was significantly correlated with increasing age (p < 0.001). Aeromonas species were more often isolated in warm seasons and in males than females (p < 0.001). Five Aeromonas species were identified. Most of the infections were from three species, namely Aeromonas veronii (52%), Aeromonas caviae (27%) and Aeromonas hydrophila (12.5%). The majority of patients with Aeromonas species infection did not have a documented overseas travel history. The findings from this study support the importance of Aeromonas species in human gastroenteritis and suggest that the sources of Aeromonas infection in Australian patients should be further investigated.

The rise in limb salvage surgeries has led to an increase in post-traumatic equinus contractures, resulting in secondary causes of tiptoe walking. There is notable evidence linking superimposed bacteremia to heightened risks of morbidity and mortality in dengue patients, particularly when the immune system is compromised, leading to documented cases of Aeromonas-induced necrotizing fasciitis with myonecrosis. A case report from August 2019 involves a five-year-old boy admitted for dengue fever, which progressed to severe bilateral leg necrotizing fasciitis caused by concurrent Aeromonas infection. The child further underwent urgent surgical debridement and post-operatively was diagnosed with necrotizing fasciomyositis of the bilateral leg. Despite urgent surgical intervention, the patient developed long-term complications of persistent equinus contractures in both legs, necessitating subsequent surgical Achilles tendon lengthening. This case underscores the potential fatality of superimposed Aeromonas septicaemia in dengue patients and emphasizes the need for vigilant monitoring and intervention in serious soft tissue infections in growing children and adolescents.

We present to you the case of 77 YO female with PMH of DM, A fib and recent H/O ascending cholangitis and cholecystectomy complicated by ruptured gallbladder with bile peritonitis and JP drain placement. After the discharge she was having diarrhea and was tested positive for C diff and was started on oral vancomycin. Almost about 2 weeks after the discharge she started to develop sudden onset of right leg pain, because of that she was taken to the ED of outside hospital where there was a suspicion of DVT, but duplex came out negative. So was send to our center for further evaluation and management where CT scan was done which showed necrotizing fasciitis. She was taken to the OR for amputation and later tissue culture came positive for Rare Aeromonas hydrophila/caviae complex.
