Factors Affecting Mortality Rates in Patients with Abdominal Vascular Injuries

Abstract: Rapid control of bleeding sites (to keep blood transfusions to < 10 units) and urgent correction of hypothermia seem to be the main factors improving survival over which the surgeon has some control.

“…The highest death rates of victims with ISS > 25 was observed in laparotomy surgery trauma patients (21) . A similar finding concerning trauma severity was observed in a study that analyzed trauma abdominal vascular injuries (11) . Furthermore, the high average of the ISS (41) was noticed in a study that analyzed 537 intraoperative deaths in eight American hospitals (5) .…”

“…The predominance of young males was clearly observed. International studies approaching general trauma victims admitted into Surgery centers (5,11) also point out this profile of patients involved in accidents or violence.…”

“…7 When a patient is in severe shock from exsanguinations caused by complex injury to the common iliac or external iliac vessels, damage control laparotomy is indicated. 8 When fecal contamination is present, there is increased risk of blowout of any type of vascular repair. Injuries to external iliac vein managed by lateral venorrhaphy can result in significant occlusion during postoperative period.…”

“…71 Haemodynamically unstable non-responders should raise suspicions of vascular injury. It has been advocated by Tyburski et al that patients with a systolic blood pressure persistently below 70mmHg undertake a prelaparotomy thoracotomy for proximal aortic control.…”

“…It has been advocated by Tyburski et al that patients with a systolic blood pressure persistently below 70mmHg undertake a prelaparotomy thoracotomy for proximal aortic control. 71 In their study, 51 patients underwent a prelaparotomy thoracotomy; 29 responded and, of those, 11 survived. Interestingly, all those who did not respond to aortic cross clamping by an increase in systolic blood pressure to at least 90mmHg died and these patients were considered irretrievable.…”
