External validation of the HIT Expert Probability (HEP) score

Abstract: The diagnosis of heparin-induced thrombocytopenia (HIT) can be challenging. The HIT Expert Probability (HEP) Score has recently been proposed to aid in the diagnosis of HIT. We sought to externally and prospectively validate the HEP score. We prospectively assessed pre-test probability of HIT for 51 consecutive patients referred to our Consultative Service for evaluation of possible HIT between August 1, 2012 and February 1, 2013. Two Vascular Medicine fellows independently applied the 4T and HEP scores for ea… Show more

“…Misdiagnosis will delay the initiation of the alternative treatment, increasing thrombotic risk and mortality [ 4 ]. Clinical scoring systems, such as the 4Ts score or the HEP score, are helpful in estimating the probability of HIT [ 5 , 6 ]. A low probability 4Ts score appears to be a robust means of excluding HIT [ 7 , 8 ] but does not rule out HIT in all cases [ 9 , 10 ] and may be difficult to apply [ 11 ].…”

“…Other probability scoring systems include the Lillo-Le Louët model, which is intended for use exclusively after CPB, and the HIT Expert Probability Score, which was developed from expert opinion. 18 , 19 …”

“…Other scoring systems have been developed, such as the HEP (HIT Expert Probability) score (Cuker et al , ). However, despite incorporating more variables, this has not been shown to be superior to the 4T's (Joseph et al , ). Given the accessibility of immunoassays, it is usually possible to combine the 4T's score with HIT assay results, to improve diagnostic accuracy (Pouplard et al , ) (Linkins et al , ; Warkentin, ; Nagler et al , ).…”
