Objective: to analyze the outcomes of arteriovenous fistula (AVF) constructed in our setup. Study Design: Retrospective observational study. Place and Duration of Study: Combined Military Hospital, Lahore, from Mar 2019 to Mar 2020. Methodology: Our study included all the patients who had their arteriovenous fistula (AVF) made during the study period. Data was collected via telephone calls, to inquire about the status of AV fistula after one year of AV fistula construction surgery. Results: Out of 130 study participants, 97 were successfully contacted. Mean age of patients was 54 .39 ± 12.75 years. Majority were male patients (71.1%) and had their first dialysis via temporary dialysis access catheter. Most of the patients (74.7%) had functional AVF. 35 (36.1%) patients were not alive at the time of contact. In 9 (9.2%) patients, AV fistula failed with the most common cause being thrombosis (77.7%). Conclusion: Current study is one of the pioneer in Pakistan where we have started to audit our performance as the dedicated access surgeons. It is high time for us to evaluate our performance so that better outcome can be offered to the dialysis dependent patients.

Introduction The objective of this study was to evaluate the effect of continuity of care on self‐management ability and quality of life (QOL) in patients undergoing maintenance hemodialysis (MHD). Methods One hundred patients were randomly assigned to the observation group and the control group. In the observation group, patients received a 12‐month continuity of care. In the control group, patients were given with routine nursing. Evaluate the patients' self‐management ability and QOL between two groups 1 week before discharge and 6 and 12 months outpatient MHD. Results Observation group had higher Hemodialysis Self‐Management Instrument (HD‐SMI) scores and Kidney Disease Quality of Life‐Short Form (KDQOL‐SF™) scores than control group at 6 and 12 months outpatient MHD. But patients in observation group had a much lower systolic blood pressure than those in control group at 12 months outpatient MHD. Conclusions Our study suggested that continuity of care in the form of online education, telephone visit, and outpatient visit could improve self‐management ability and QOL of patients undergoing MHD.

Dispositional Optimism and Context Sensitivity: Psychological Contributors to Frailty Status Among Elderly Outpatients

The association of resilience-related factors with frailty is a recent research topic. Dispositional optimism and context sensitivity are two psychological factors that differently contribute to individual resilience. This study aimed at investigating whether dispositional optimism and context sensitivity might contribute to a multifactorial model of frailty, together with established relevant factors such as cognitive and physical factors. This cross-sectional study involved 141 elderly outpatients (42 males and 99 females) aged ≥65 years, who were referred to the Geriatrics and Multidimensional Evaluation Clinic of the University Hospital of Messina. We used the following measures: the Mini-Mental State Examination (MMSE) to screen for global cognitive functioning; 4-m gait speed and handgrip strength to measure physical performance; a 35-item Frailty Index (FI) to evaluate patients’ frailty status; the revised Life Orientation Test (LOT-R) to gauge dispositional optimism; and the Context Sensitivity Index (CSI) to measure context sensitivity. We found that LOT-R (β = −0.190, p = 0.038), CSI (β = −0.191, p = 0.035), and MMSE (β = −0.466, p < 0.001) were all significantly associated with FI. Gait speed was only marginally associated with FI (β = −0.184, p = 0.053). The present study showed a novel association of dispositional optimism and context sensitivity with frailty among elderly outpatients. These preliminary findings support a multidimensional approach to frailty in which even peculiar psychological features might provide a significant contribution.
