Acute intestinal infections (AII) remain one of the most important problems of children’s infectious pathology, as this disease is diagnosed with severe forms and the mortality remains in high rate. Among patients with acute intestinal infection, children account for 60-70%. Severe forms of the disease are still noted to lead to death in young children. In addition, in recent years, there has been an increase in acute intestinal infections, an increase in their clinical manifestations and a long-standing uneven trend. The severity of the disease often occurs due to the development of viral-bacterial and bacterial-bacterial Complex in young children. V.F. Uchaikina et al. According to (2004), the main criteria for diagnosis include the following forms: gastritis, enteritis, gastroenteritis, colitis, enterocolitis, gastroenterocolitis, distal colitis. In the initial period of the disease (until the results of laboratory tests are obtained), for the construction of rational etiotropic and pathogenetic-based therapy of the OII, it is necessary to first determine the pathological process(enteritis, colitis) by taking into account the severity of the disease (mild, moderate, severe). Even with the severe course of the disease in childhood, acute intestinal infections contribute to their formation, especially in children with a decrease in the health index, prone to other infectious and somatic diseases. About the role of OIC in the pathogenesis of chronic diseases of the digestive system, leading to overeating and decompensation of diseases of the gastrointestinal tract. At present, the etiological structure of the AII has changed. Now the leading role among them in the world is undoubtedly that 50-80% of AII in children belong to viruses. Currently, intestinal infections are associated with representatives of at least 8 different types of viral oylases: Reoviridae, Saliceviridae, Adenoviridae, Astroviridae, Picornoviridae, Cobovirus, Coronaviridae, ovoviridae, Picornaviridae agent. The etiology of viral diarrhea is similar in developed and developing countries. The spectrum of possible etiological agents of acute gastroenteritis continues to increase, since previously unknown viruses were detected in patients, but it is necessary to confirm their significance in this pathology. This article aims to study the etiotropic treatment measures aimed at inhibiting the development of acute intestinal infection bacteria or virus.

Pneumococcal infection remains a significant global health problem, and vaccination is the main measure for its prevention. To date, the period of use of pneumococcal conjugated polysaccharide vaccines in Russia exceeds 14 years, and 13-valent conjugated polysaccharide pneumococcal vaccine (PCV13) - more than 10 years. During this time, extensive experience has been accumulated in the use of this type of vaccines, and many studies have been carried out to evaluate their effectiveness and safety.The purpose of this review is to summarize the experience of using PCV13 in Russian Federation with an assessment of its epidemiological and clinical effectiveness. A search was made for scientific publications devoted to the study of the epidemiological efficacy, the safety as well as cost-effectiveness of PCV13 use in Russian Federation. The review included original studies published in Russian journals. The results of the studies carried out indicate the efficacy and safety of PCV13 for both adults and children. The effectiveness of immunization of children at risk (premature, suffering from congenital pathology, having chronic diseases and often ill) was demonstrated, the need and safety of the timely start of vaccination (from 2 months of age) of newborns was shown, the possibility of its combination with immunization against other infections within the framework of the national vaccination schedule, the importance of following the recommended vaccination schedule in accordance with the age of the child. The effectiveness of vaccination of adults suffering from chronic diseases has been shown both in terms of preventing the aggravation of the course of the underlying pathology and reducing the risk of pneumonia. Positive experience has been gained in immunizing adults from occupational risk groups - medical workers, conscripts and persons exposed to a harmful production factor and having occupational lung diseases. The conducted studies have shown a high cost-effectiveness of PCV13 vaccination, however, with any changes in price and epidemiological parameters, it is necessary to clarify the economic feasibility of vaccination under the changed conditions. Taking into account the positive experience gained in immunization, it seems appropriate to further maintain a high level of vaccination coverage of the child population, expanding risk groups among the adult population subject to vaccination against pneumococcal infection within the framework of the National Immunization Schedule, taking into account its epidemiological, clinical and economic efficiency.

Immunological Risk Factors for Community-Acquired Pneumonia in Chrysotile Asbestos Workers

Introduction: The paper presents the results of a retrospective analysis of the incidence rates of community-acquired pneumonia in chrysotile miners and millers in comparison with the adult population of a municipality. It also demonstrates findings of the study of some immune response parameters in Streptococcus pneumoniae carriers among chrysotile asbestos workers. The objective of the research was to study specifics of the development of community-acquired pneumonia and to determine immune indices in healthy workers (Streptococcus pneumoniae carriers) occupationally exposed to chrysotile asbestos. Materials and methods: First, we analyzed community-acquired pneumonia incidence rates in chrysotile asbestos workers. For the retrospective analysis, we extracted comprehensive emergency notifications about cases of infectious disease, food poisoning, acute occupational poisoning, community-acquired pneumonia, and vaccine adverse reactions (Form 058/u) from the Epidemiologic Surveillance Information System maintained by the Sverdlovsk Regional Center for Hygiene and Epidemiology and Federal Statistics Form No. 2 “Information about Infectious and Parasitic Diseases” in the Sverdlovsk Region. We defined jobs and formed three groups of workers with high incidence rates of community-acquired pneumonia: group 1 included workers involved in extraction and transportation of chrysotile asbestos, group 2 consisted of workers of the ore-dressing factory while group 3 included workers of auxiliary subdivisions (logistics, security, administration, central automated control station). In 2011, 2012, 2013, and 2019, the incidence rate of community-acquired pneumonia in workers was significantly higher. The annual increase had been observed since 2016 and reached its maximum of 1,032.97±158.88 in 2019 exceeding the long-term average rate in the industry and the town by 12.77 % and 15 %, respectively. We assessed the immune status and performed a comparative analysis of immunological indices in 143 workers of selected jobs, both carriers and noncarriers of Streptococcus pneumoniae. The carriers demonstrated a significant increase in the production immunoglobulin E indicating general sensitization and immunoglobulin G, a significantly lower relative count of mature T cells, and a higher lymphocyte count in the populations of natural killer cells.
