Introduction: This study examined the effect of a new bioactive dentin substitute material (Biodentine) on the viability of human gingival fibroblasts. Methods: Biodentine, White ProRoot mineral trioxide aggregate (MTA), and glass ionomer cement were evaluated. Human gingival fibroblasts were incubated for 1, 3, and 7 days both in the extracts from immersion of set materials in culture medium and directly on the surface of the set materials immersed in culture medium. Fibroblasts cultured in Dulbecco modified Eagle medium were used as a control group. Cytotoxicity was evaluated by flow cytometry, and the adhesion of human gingival fibroblasts to the surface of the set materials was assessed by using scanning electron microscopy. The data of cell cytotoxicity were analyzed statistically by using a one-way analysis of variance test at a significance level of P < .05. Results: Cells exposed to extracts from Biodentine and MTA showed the highest viabilities at all extract concentrations, whereas cells exposed to glass ionomer cement extracts displayed the lowest viabilities (P < .05). There was no significant difference in cell viabilities between Biodentine and MTA during the entire experimental period (P > .05). Human gingival fibroblasts in contact with Biodentine and MTA attached to and spread over the material surface after an overnight culture and increased in numbers after 3 and 7 days of culture. Conclusions: Biodentine caused gingival fibroblast reaction similar to that by MTA. Both materials were less cytotoxic than glass ionomer cement. (J Endod 2013;39:478-483)

Hydraulische calciumsilicaatcementen (HCSC's) zijn materialen die sinds de introductie van 'mineral trioxide aggregate' (MTA) in de jaren 90 van de vorige eeuw aan populariteit hebben gewonnen binnen de endodontologie. MTA is Portlandcement waaraan bismutoxide is toegevoegd om de radiopaciteit te verhogen. Het belang van MTA is de eigenschap om in water en in een vochtige omgeving te kunnen uitharden. Desondanks heeft MTA ook belangrijke beperkingen, zoals gebruiksonvriendelijkheid en het risico van verkleuring van het gebitselement. Recent zijn er talloze producten die gebaseerd zijn op de chemie van HCSC's, en dus te beschouwen als modificaties van MTA, op de markt gebracht met de bedoeling de beperkingen van MTA te verminderen. Ondanks de potentiële voordelen van deze materialen, hebben ze elk hun specifieke beperkingen die nog onvoldoende gekend en onderzocht zijn. InleidingDe endodontische behandeling heeft als doel het vormgeven en het desinfecteren van het wortelkanaalstelsel om gunstige omstandigheden te creëren voor de genezing van laesies van endodontische oorsprong (Thoden van Velzen en Wesselink, 2010). Om deze situatie ook op de lange termijn te behouden wordt de plaatsing van een wortelkanaalvulling met langdurige afsluitende eigenschappen gunstig geacht voor de effectiviteit van de behandeling (Waltimo et al, 2005). De endodontische behandeling wordt uitgevoerd onder isolatie van rubberdam om contaminatie te voorkomen en een droge omgeving te creëren. De afsluitende eigenschappen van de gebruikte wortelkanaalvulmaterialen komen immers het beste tot hun recht als ze onder de juiste omstandigheden worden aangebracht.Er bestaan echter specifieke klinische situaties waarin een teveel aan vocht de volledige uitharding van conventionele vulmaterialen kan hinderen, waardoor hun eigenschappen negatief worden beïnvloed. Voorbeelden van dergelijke klinische situaties zijn het aanbrengen van een retrograde vulling bij apicale chirurgie, het repareren van een perforatie, de directe pulpaoverkapping en het behandelen van open apices. Om deze specifieke situaties te kunnen behandelen werd in de jaren 90 van de vorige eeuw een mengsel van Portlandcement en bismutoxide (toegevoegd voor radiopaciteit) in de endodontologie geïntrodu-ceerd en gepatenteerd als 'mineral trioxide aggregate' (MTA) (Torabinejad en White, 1995). Na deze ontdekking werden verschillende materialen ontwikkeld die leken op de originele samenstelling van MTA, maar gebaseerd waren op tricalcium-en dicalciumsilicaten. Deze familie van materialen is het beste bekend als 'hydraulische calciumsilicaatcementen (HCSC) (Niu et al, 2014). HCSC's zijn hydraulische materialen, dit betekent dat de uithardingsreactie plaatsvindt wanneer ze gemengd worden met water en dat ze uit kunnen harden in een vochtige omgeving.Dit overzichtsartikel zal eerst ingaan op de fysicochemische en biologische eigenschappen alsmede de belangrijkste beperkingen van MTA, aangezien van dat materiaal de voornaamste kenmerken en eigenschappen gelijk zijn aan die van de verschill...
