Abstract: IntroductionChagas disease is one of the most important endemic parasitic diseases in Latin America. In its chronic phase, progression to cardiomyopathy has high morbidity and mortality. The persistence of a normal electrocardiogram (ECG) provides a similar prognosis to that of a non-diseased population. Benznidazole (BNZ) is the only drug with trypanocidal action available in Brazil.Materials/Methods/ResultsA group of 310 patients with chronic Chagas disease who had normal ECGs at the first medical visit perf… Show more

“…Available treatment (NFX and BZL) for this stage of the disease is controversial (1,4,16). Several studies showed that antiparasitic treatment reduces the parasite load in the bloodstream (16,41,42). Furthermore, two recent studies showed that the efficacy of treatment depends on the degree to which the patient is compromised.…”

“…Thus, the large Benznidazole Evaluation for Interrupting Trypanosomiasis (BENEFIT) study done in patients with established myocardiopathy showed that, despite elimination of the parasite, there was no improvement in the cardiac condition (16). In contrast, BZL treatment also was described to prevent the occurrence of electrocardiographic alterations in individuals without mild cardiac compromise (asymptomatic) (41). Antiparasitic treatment was demonstrated to induce a substantial activation of T cells and change in the T cell response related to the loss of the capacity for cytokine production by T cells after treatment (25,43).…”

“…This reduction in parasite load is heavily dependent on geographic site, and parasite strain. Due to the above evidence, the current trend is to treat cases of Chagas disease without heart complications 8 . The major disadvantage of both drugs is their toxicity profi les [9][10][11][12] .…”

“…As previously documented, a decrease in patient serological titers against several specific parasite antigens is associated with a substantial reduction in the parasitic load and with an improvement in the clinical status of treated Chagas disease patients (38,39). Therefore, the evaluation of the reactivity against these antigens could represent a useful serological marker system for monitoring the effectiveness of drug treatment in Chagas disease patients and a tool that could allow the early detection of therapeutic failure.…”
