Background Débridement and bone marrow stimulation is an effective treatment option for patients with talar osteochondral defects. However, whether surgical factors affect the success of microfracture treatment of talar osteochondral defects is not well characterized. Questions/purposes We hypothesized (1) holes that reach deeper into the bone marrow-filled trabecular bone allow for more hyaline-like repair; and (2) a larger number of holes with a smaller diameter result in more solid integration of the repair tissue, less need for new bone formation, and higher fill of the defect. Methods Talar osteochondral defects that were 6 mm in diameter were drilled bilaterally in 16 goats (32 samples). In eight goats, one defect was treated by drilling six 0.45-mm diameter holes in the defect 2 mm deep; in the remaining eight goats, six 0.45-mm diameter holes were punctured to a depth of 4 mm. All contralateral defects were treated with three 1.1-mm diameter holes 3 mm deep, mimicking the clinical situation, as internal controls. After 24 weeks, histologic analyses were performed using Masson-Goldner/Safranin-O sections scored using a modified O'Driscoll histologic score (scale, 0-22) and analyzed for osteoid deposition. Purpose:To correlate short and long T2* water fractions, derived from ultrashort-echo time (TE) magnetic resonance (MR) imaging, with semiquantitative histopathologic and polarized light microscopic (PLM) assessment of human cadaveric patellae cartilage. Materials and Methods:Twenty human cadaveric patellae were evaluated by using ultrashort-TE imaging, spin-echo imaging, histopathologic analysis, and PLM, with institutional review board approval. Short and long T2* water components were evaluated for each patella by using bicomponent fitting of ultrashort-TE signal decay. Four to six regions of interest (ROIs) within each patella were chosen for correlation between ultrashort-TE bicomponent analysis, histopathologic grading (Mankin score), and PLM grading (Vaudey score). Results:Ultrashort-TE imaging with bicomponent analysis showed two distinct water components with a short T2* and a longer T2* in all patellae. ROI analysis showed that the short T2* fraction was correlated significantly with the Mankin (r = 0.66, P , .001) and Vaudey (r = 0.68, P , .001) scores. The Mankin scores were weakly positively correlated with T2 (r = 0.28, P = .13) and short T2* (r = 0.24, P = .14) but were negatively correlated with long T2* (r = 20.55, P , .01). The Vaudey scores were weakly positively correlated with T2 (r = 0.18, P = .16) and short T2* (r = 0.22, P = .14) but were negatively correlated with long T2* (r = 20.55, P , .01). Conclusion:Short T2* water fraction derived from ultrashort-TE imaging with bicomponent analysis correlates significantly with both the Mankin and Vaudey scores and may serve as a biomarker of cartilage degeneration.q RSNA, 2012

E-mail: wilmavet@gmail.comThe aim of this study was to evaluate the progression of lesions in different stages of osteoarthritis (OA) experimental by radiography (RX), computed tomography (CT), macroscopic and histopathology, linking these different diagnostic methods, helped to provide information that helps the best time for the therapeutic approach. Four experimental periods were delineated at 3, 6, 9 and 12 weeks after induction of OA, known as PI, PII, PIII and PIV, respectively, each with six animals. We evaluated the five compartments of the femorotibial joint: medial femoral condyle (MFC), lateral femoral condyle (LFC), medial tibial plateau (MTP), lateral tibial plateau (LTP) and femoral trochlea (FT). Therefore we established an index by compartment (IC) and by adding such an index was estimated joint femorotibial (IFT). It was observed that the CFM was the compartment with the highest IC also differed significantly (p<0.05) from other compartments. Compartments showed no significant difference (p>0.05) between the PI and PII, however contrary fact occurred between the PII and PIII (p<0.05), PIII and PIV (p<0.01) and between PI and PIV (p<0.001). Similarly the IFT, showed a significant difference in the animals of PIV compared to PI (p<0.001), PII (p<0.001) and PIII (p<0.01), and there was no statistical difference (p> 0.05) between the PI and PII. In the variation of the average interval between periods, there was a higher value between the PIII PIV and for the other intervals of time periods (PI, PII, and PIII-PII). However, these intervals showed no statistically significant difference (p>0.05). Through the RX, CT, macroscopic and histopathological findings, we found similar patterns among individuals within the same period demonstrating a gradual progression of the disease. These results show that between 3 and 6 weeks progression of the lesion is slower and probably also can be reversed in comparison to other ranges where proved further progression between 9 and 12 weeks after induction of trauma OA. These results may provide a better therapeutic approach aimed at reversing the lesions in early stages of OA. We conclude that the interconnection of the four diagnostic methods individually classified into scores, which were unified in both indices in the evaluation by the femorotibial joint compartment and may represent a diagnostic condition closer to the true condition of the injury and its progression. .
