Background: Diabetes mellitus is a metabolic chronic disease affecting a majority of adults with associated complications. The non-compliance to the anti-diabetic medication has become a global challenge to achieving optimal glucose control among Diabetes Type 2 patients. This study, therefore, sought to determine the self-care practices and drivers of anti-diabetic type 2 medication non-compliance among patients accessing health care at Volta River Authority Hospital in Akosombo of Ghana. Methods: A quantitative study and descriptive cross-sectional design employed a simple random sampling technique to recruit 220 diabetes type 2 patients by administering a structured questionnaire face to face to gather data. Data were analysed with the help of Stata 16. A descriptive and inferential statistic was conducted to determine the relationship between the dependent and independent variables at a 95% confidence interval and a p-value of less than 0.05 was considered statically significant. Results: The study found that non-adherence to anti-diabetic medication was 45.5%. Most of the participants had inadequate (52.3%) self-care practices. Age of participants (40-49) years [p=0.0001], Female [p=0.004] significantly influenced non-compliance. Being Single [p=0.001] and presence of comorbidity [p=0.001] also associated with medication non-compliance. Conclusion: The study concluded that a significant proportion of diabetes type 2 patients do not comply with anti-diabetic medication and this was influenced by inadequate self-care practices, age, female, comorbidity and being single. The study recommended the intensification of awareness creation on complications of non-complying to anti-diabetic medication and education on self-care practices through mass media. Further studies are required to identify the possible predictors of inadequate self-care practices that influence anti-diabetic medication non-compliance.

BACKGROUND: Therapeutic observance remains a major problem in managing diabetic subjects, just like in other pathologies treated by medication and lifestyle modification. This study aims to determine the rate of therapeutic observance among diabetic subjects in the province of Essaouira and to identify the factors that influence it.METHODS: A cross-sectional questionnaire survey was conducted among 498 type 1 and 2 diabetic subjects, regularly being checked at different health centers in the province of Essaouira (Morocco).RESULTS: Almost a quarter of the surveyed subjects (23.3%) had poor observance. The results of the binary logistic regression model show that, in order of importance, observance is associated with six factors: Availability and access to medical treatment (Odds Ratio OR: 3 ; 95% CI confidence interval [1.78-5.03]); the side effects related to thetreatment (OR: 2.60 ; 95% CI [1.65-4.09]); the family support (OR:1.58 ; 95% CI [0.95-2.61]); duration of diabetes (OR: 0.55 ; 95% CI [0.34-0.88]); the age (OR: 0.50 ; 95% CI [0.30-0.82]); awareness level about the disease (OR: 0.43 ; 95% CI [0.21-0.90])CONCLUSION: The results of the present study have allowed us to identify several factors that can influence therapeutic observance, that prove necessary to be considered and acted upon.

Pharmaceutical care is a relevant service to promote the rational drug use and pharmacotherapeutic success, due to the pharmacist's knowledge and accessibility. On this regard, there is a great relevance on providing this service to pregnant women, due to the possible maternal-fetal risks associated with some drugs. As well as pre-existing diseases and conditions for instance, arterial hypertension and gestational diabetes mellitus which require the use of pharmacological therapies and are of great relevance in Brazilian public health. Due to the high incidence and the complications they may cause, it is an opportune period for the pharmaceutical care to assist treatment adherence and consequently achieve therapeutic success. In this context, the aim of this study is to understand the importance of clinical pharmacists in the pharmacological care during pregnancy. A research was carried out in the databases Scielo, Google Scholar, PubMed and ScienceDirect databases, using the descriptors: risk categories; hypertension; diabetes mellitus; self-medication; pregnancy; nausea; vomiting; eclampsia and pharmaceutical assistance, all selected from DeCS in the languages: Portuguese and English. The importance of pharmaceutical care in pregnancy was evidenced through this study regarding not only the safety and adequate guidance to the use of drug therapies to reduce self-medication, but also the therapeutic adherence process through continuous and regular monitoring. Hence, contributing to the improvement of both maternal and fetal life quality by the pharmaceutical care that highlights whenever necessary the importance of correct treatment compliance and advises the need of therapeutic interventions. Therefore, this study has a unique relevance once it explained the pharmaceutical care importance for pregnant women. Furthermore, it collaborates for the development of plans and actions that aim to expose and conscientize the importance of the clinical pharmacist during this period.
