Background Among upper limb injuries, carpal bone fractures and dislocation, Essex-Lopresti injury, and the terrible triad injury of the elbow are serious and relatively rare injuries. These injuries require surgical intervention. The surgical method is difficult, and the treatment effect is poor. These injuries have not been described in the same limb in the literature. Case presentation A 21-year-old male patient fell from a height in our institution and sustained multiple injuries, including carpal bone fracture-dislocation, Essex-Lopresti injury, and the terrible triad injury of the elbow of his right upper limb. After 2 surgeries and rehabilitation, he returned to work. We reviewed available reviews and related literature on serious upper-limb damage. Conclusions Full-length forearm injury is very rarely encountered, and the management of such fractures is difficult. Radial head replacement with a metal prosthesis, reconstructed the IOM with Tightrope, and fixed the DRUJ with a K-wire pin is appropriate treatment.

Фетальна топографія нервів ліктьової ділянки актуальність. розширення показань до хірургічних утручань на структурах ліктьової ділянки зумовлює необхідність цілеспрямованого вивчення індивідуальних та вікових особливостей іннервації шкіри ліктьової ділянки і капсули ліктьового суглоба.Мета. з'ясувати джерела іннервації шкіри ліктьової ділянки і суглобової капсули ліктьового суглоба у плодів людини 4-10 місяців.Методи. Макромікроскопічне дослідження проведено на препаратах верхніх кінцівок 26 плодів людини 81,0-375,0 мм тім'янокуприкової довжини (ткд).результати. у більшості досліджених плодів спостерігався класичний варіант топографії нервів ліктьової ділянки. зазвичай іннервацію ліктьового суглоба здійснюють суглобові гілки серединного, ліктьового і променевого нервів. у плода 225,0 мм ткд спостерігався атиповий варіант топографії лівого присереднього шкірного нерва передпліччя, що розгалужувався на 3 гілки: бічну, проміжну та присередню.в одному випадку (плід 260,0 мм ткд) виявлено зв'язок між присередньою гілкою правого м'язово-шкірного нерва та правим присереднім шкірним нервом плеча.у плода 170,0 мм ткд лівий присередній шкірний нерв плеча був відсутній, у цьому випадку чутливу іннервацію шкіри передньоприсередньої поверхні плеча забезпечував лівий присередній шкірний нерв передпліччя, що може свідчити про компенсаторний розвиток його гілок.висновки. у плодів людини встановлено варіабельність кількості та джерел походження гілок, що забезпечують іннервацію капсули ліктьового суглоба та шкіри ліктьової ділянки.вважаємо, що зона порушення чутливості при ураженні шкірних нервів плечової і ліктьової ділянок, і ділянки передпліччя, як правило, менша, ніж анатомічні території розповсюдження шкірних нервів, або їхніх гілок. це пов'язано з тим, що окремі ділянки шкіри ліктьової ділянки отримують додаткову іннервацію від суміжних нервів -"зони перекриття". описані варіанти індивідуальної та вікової анатомічної мінливості нервів ліктьової ділянки у плодів людини сприятимуть удосконаленню хірургічної тактики проведення реконструктивних операцій при травматичних пошкодженнях довгих гілок плечового сплетення.ключові слова: ліктьова ділянка, плечове сплетення, ліктьовий суглоб, іннервація, плід, людина.

Treatment of Supracondylar Humerus Fracture in Adults by Parallel Plates

Objective: The aim of this study was to evaluate the outcome of parallel plates for the treatment of supracondylar humerus fractures in adults. Patients and Methods: This study included 20 adult patients with supracondylar humerus fractures. They were treated by open reduction and internal fixation by parallel plates through olecranon osteotomy technique. They were followed up clinically and radiologically for 6 months. On admission, all were assessed by careful history taking and detailed clinical examination. The preoperative radiological workup was performed by antero-posterior and lateral radiographs of the elbow with traction and computed tomography was performed with three-dimensional reconstruction. Postoperative, patients were assessed after 2, 4, 8, 12 weeks and 6 months of surgery, with clinical and radiological evaluations and assessed for pain, swelling, range of joint motion and radiological union. The functional assessment of the patient was done according to Mayo elbow performance index and the outcome was graded into excellent, good, poor and fair. RESULTS: The excellent outcome was achieved among 55.0%, while 30.0% had good, 10% had fair and 5.0% had poor outcome. The overall complication rate was 30.0% and the commonest were pain and stiffness (15.0% for each). The mean±SD times to clinical and radiological union were 12.75 ± 6.44 and 14.70 ± 7.84 weeks, respectively. The excellent outcome was significantly associated with lower rate of complications and shorter time to clinical and radiological union. CONCLUSION: Parallel plates for the treatment of adult supracondylar humerus fractures could be considered safe and effective technique, leading to good functional outcome and a complication rate comparable to published literature.
