Aim We aimed to translate and cross-culturally adapt the International Knee Documentation Committee—subjective knee form (IKDC) in Romanian. Method The original (US) IKDC—subjective knee form was translated according to recommended guidelines. Validity was tested using Spearmans’s correlation coefficient between score sand test-retest reproducibility. Reliability and internal consistency were determined using Cronbach’s alpha coefficient and intraclass correlation coefficient (ICC). Results A total of 106 data sets were available for processing. The average age was 52 years and the male to female ratio was 40:66. Fifty-five subjects repeated the form after an average of 4 days. There were no floor or ceiling effects (range 3.4–74.7). There was a strong correlation between the first and repeated administration of the IKDC—subjective knee form (r = 0.816, n = 50) and moderate compared to Tegner-Lysholm knee rating scale (r = 0.506, n = 102), KOOSJR (Knee disability and Osteoarthritis Outcome Score for Joint Replacement, r = − 0.622, n = 96), EuroqolEQ-5D-5L Index (r = 0.633, n = 100) and visual analogue scale VAS (r = 0.484, n = 99). Internal consistency was moderate with Cronbach’s alpha 0.611 (n = 102) and ICC 0.611 for average measures (95% CI 0.493–0.713). Conclusion The Romanian translation of the IKDC—subjective knee form is a valid, consistent and reproducible outcome measure in patients with knee pain and dysfunction.

Degeneration of articular cartilage (AC) is actively studied in knee osteoarthritis (OA) research via magnetic resonance imaging (MRI). Segmentation of AC tissues from MRI data is an essential step in quantification of their damage. Deep learning (DL) based methods have shown potential in this realm and are the current state-of-the-art, however, their robustness to heterogeneity of MRI acquisition settings remains an open problem. In this study, we investigated two modern regularization techniques -mixup and adversarial unsupervised domain adaptation (UDA) -to improve the robustness of DL-based knee cartilage segmentation to new MRI acquisition settings. Our validation setup included two datasets produced by different MRI scanners and using distinct data acquisition protocols. We assessed the robustness of automatic segmentation by comparing mixup and UDA approaches to a strong baseline method at different OA severity stages and, additionally, in relation to anatomical locations. Our results showed that for moderate changes in knee MRI data acquisition settings both approaches may provide notable improvements in the robustness, which are consistent for all stages of the disease and affect the clinically important areas of the knee joint. However, mixup may be considered as a recommended approach, since it is more computationally efficient and does not require additional data from the target acquisition setup.

Background: The prevalence of minute structural abnormalities in early knee osteoarthritis (KOA) and their association with knee symptoms remains unclear. We investigated the prevalence of abnormalities detected by magnetic resonance imaging (MRI) and their relation to knee symptoms in women without radiographic evidence of KOA.Methods: This cross-sectional cohort study in a Japanese population included 261 women without radiographic evidence of KOA, which was defined as a Kellgren‒Lawrence grade < 2. All participants underwent T2-weighted fat-suppressed MRI of their right knee. Structural abnormalities (cartilage damage, bone marrow lesions [BMLs], subchondral cysts, bone attrition, osteophytes, meniscal lesions, and synovitis) were scored according to the Whole-Organ MRI Scoring method. Knee symptoms were evaluated by the Knee injury and Osteoarthritis and Outcome Score (KOOS). Participants were divided into early KOA and non-KOA groups based on early KOA classification criteria. Logistic regression analysis was performed to evaluate the relationship between MRI abnormalities and knee symptoms.Results: Cartilage damage were the most common abnormality (53%), followed by osteophytes (43%), and BMLs (33%). The prevalence of BMLs (47%), meniscal lesions (26%), and synovitis (37%) was higher in early KOA than in non-KOA. On logistic regression analysis, synovitis (odds ratio [OR] = 1.901, P = 0.013) and meniscal lesions (OR = 1.410, P = 0.045) were positively associated with the presence of early KOA.Conclusions: The prevalence of BMLs, meniscal lesions, and synovitis was higher in symptomatic knees. Synovitis was most strongly associated with knee pain and might be a therapeutic target in patients with early KOA.
