Introduction: Systemic lupus erythematosus (SLE) is considered an autoimmune disorder that influence many organs, distinguished by complement activation, synthesis of many autoantibodies, organ damage, and deposition of immune complex.Objective: Examine the relationship between the interferon regulatory factor 5 (IRF5) gene's genetic polymorphism and lupus nephritis disease activity as well as the correlation between SLE and these conditions. Patients and methods: Sixty-three children were included and classified into three groups as follows: 21 children with lupus nephritis (LN), 21 lupus children without nephritis, and 21 control cases. Blood samples from all children were tested for the IRF5 gene (rs2004640) polymorphism utilizing the Polymerase Chain Reaction-Restriction Fragment Length Polymorphism (PCR-RFLP) method. Results: Cases and controls differed in high statistically significant way regarding the frequency of TT genotype (64.3% and 23.8% respectively with p value <0.001) and frequency of risk allele T (77.4% and 38.1% respectively with p <0.001). However, when it came to IRF5 genotypes and alleles, there was no statistically significant difference between those with and without nephritis. Conclusion:Children in Egypt who have the rs2004640 T allele and TT genotype may be at increased risk of developing SLE. Egyptian children with SLE who carry the rs2004640 T allele or TT genotype do not have an increased risk of developing nephritis.

Background: Increasing studies in the last decade have led to the widespread understanding that C4d, a split product of complement component 4 (C4), is a potential biomarker for systemic lupus erythematosus (SLE) and lupus nephritis (LN). Purpose: The aim of this review is to summarize the highlights of studies investigating the use of C4d as a biomarker for diagnosing and monitoring SLE and LN patients. Data collection: we searched PubMed/Medline and Wanfang databases using the terms “C4d and systemic lupus erythematosus”, “C4d and lupus nephritis”, and “Complement C4d”. Results: The deposition of C4d on circulating blood cells has been shown in several clinical studies to be a potential diagnostic marker that can be used to monitor patients with SLE. In addition, C4d deposits on circulating blood cells may be a helpful diagnostic marker for LN, one of the most severe complications of SLE. Meanwhile, studies utilizing renal biopsy specimens have indicated that C4d deposition in the renal peritubular capillaries of LN patients may predict more severe LN or a worse patient prognosis. Generally, a high plasma C4d level and a high plasma C4d/C4 ratio may also be promising indicators that can be used to monitor patients with SLE and LN. Conclusions: C4d detection may be a novel strategy for further clinical prediction and therapy.

Systemic Lupus Erythematosus (SLE) cases in pediatric are uncommon, compared to adult patients, who are mostly undetected at primary healthcare, especially with subtle clinical manifestations. Detection is necessary as early as possible so that further progressivity can be controlled. A 12-year-old child came with a rash on the face confirmed by a physical examination as malar rash and discoid rash. Laboratory results showed a positive antinuclear antibody (ANA) and double-stranded DNA (dsDNA) while complete blood count, liver function test, kidney function test and urinalysis were found within normal limits. Based on those findings we conclude that the diagnosis was SLE with subtle clinical manifestation. Evaluation needs to be done at least every 6 months as a scoring of the activity of disease and organ damage. The evaluation includes signs and symptoms, laboratory work-up such as complete blood count, organ function test, urinalysis, and immunologic parameters.
