Erucismo por Lonomia spp em Teresópolis, RJ, Brasil: relato de um caso provável e revisão da literatura

Abstract: This is a case report of a 44-year-old male living in Teresópolis, RJ, Brazil, probably poisoned by contact with a Lonomia caterpillar, who presented hemolytic anemia, decreased platelet count and acute renal insufficiency. Lonomia erucism diagnosis was established by anamnesis and clinical and laboratory manifestations. Therapeutic measures consisting of hemotransfusion and hemodialysis were successful. Physiopathologic and clinical features of erucism by Lonomia are discussed.

“…Its N-terminal sequence suggests that this toxin belongs to group III PLA 2 [74] . Although intravascular hemolysis has been described in vitro, this condition is rare in humans [16,21] . Differently to what is observed in human victims [12,15,20,29] , experimentally envenomed rats develop intravascular hemolysis with hemoglobinuria but not hematuria [73] whereas in humans hematuria can be observed [10,13] .…”

“…Bleeding diathesis may occur a few hours after the accident, which consists of gengivorrhagia, ecchymosis and hematomas of variable intensity, bleeding from many sites (nose, gums, gut, genitourinary tract, and even from recent wounds), epistaxis, hematemesis or hematuria [10,13,15] . Hemolysis has been described in only two cases out of all envenomed patients in southern Brazil [16,21] . In the most severe cases, if the victim is not quickly treated, pulmonary and intracranial hemorrhages and acute renal failure frequently lead to death [12,13,20,[22][23][24] .…”

“…A case of erucism reported in Teresópolis municipality (Rio de Janeiro State), located approximately 120km from Juiz de Fora, presented symptomatology characteristic of accidents with specimens of Lonomia (Corrêa et al 2004). Although located in different states, both cities are nearby and present a very similar climate and vegetal covering.…”
