Epidemiology of lactic acidosis in type 2 diabetes patients with metformin in Japan

Abstract: PurposeTo estimate the incidence of lactic acidosis (LA) and role of metformin in Japanese patients with type 2 diabetes mellitus (T2DM) treated with anti‐diabetes drugs.MethodsThis retrospective propensity score matched cohort study was conducted using the Japanese Medical Data Vision claims database. T2DM patients aged 18 or above who received diabetes drugs during January 2010 through August 2014 were identified. Cases of LA were identified based on reimbursement codes and confirmed by lactic acid test and … Show more

“…Lactate levels are significantly higher in ICU diabetics with or without septic shock (Additional file 1 : Figure S4). This issue still remains controversial with studies finding no effect of metformin on lactate rate [ 5 , 15 , 16 ] or, on the contrary, finding an increased lactate [ 17 27 ]. One reason for this discrepancy may be that ICU patients, unlike other patients, suffer acute stress with endogenous catecholamine release leading to increased lactate levels through adrenergic receptor stimulation.…”

“…Costs that occur outside this hospital network are not captured. The database had been used in health economic or epidemiological analyses of many different indications such as schizophrenia [ 15 ], influenza [ 16 ], infectious diseases [ 17 ], RA [ 18 , 19 ], cardiovascular disease [ 20 ], diabetes [ 21 ], and prostate cancer [ 22 , 23 ]. The time span of our analysis was from March 2009 to September 2015.…”

“…With regard to the association of metformin with lactic acidosis, unlike previous studies [11,12], the present study did not find any difference in the incidence rate of lactic acidosis between patients treated with metformin and those treated with other antidiabetic drugs. Several studies have indicated that complications of T2D, such as renal diseases, increase the risk of lactic acidosis in patients receiving metformin administration [16,17,22]. In the package inserts of metformin, the use of metformin is contraindicated for those patients having a high risk of incidence of lactic acidosis, i.e.…”
