Enophthalmos as a Complication of Rhinoplasty

Abstract: Silent sinus syndrome is a known clinical entity caused by negative sinus pressure from acquired obstruction of the maxillary sinus ostium with resulting enophthalmos and hypoglobus. We present a case in which, after septorhinoplasty, a patient developed progressive enophthalmos and hypoglobus. His evaluation was consistent with silent sinus syndrome. This case illustrates the complex reconstructive challenges of repairing an orbit in the setting of fistulization of the orbit with the maxillary sinus cavity. I… Show more

“…Most reported cases of SSS occur spontaneously, 3 although SSS has been described following endoscopic sinus surgery and open cosmetic septorhinoplasty. 4,5 In this case, we hypothesize that during lateral fracture of the inferior turbinate, inadvertent lateral fracture of the uncinate process or perhaps even fracture of the medial maxillary wall might have caused obstruction of the middle meatus, ultimately leading to SSS.…”

“…Both aesthetic and non-aesthetic complications of rhinoplasty have been reported. [1][2][3] Post-rhinoplasty infection of the nose is a much less frequently encountered complication, and is not well documented. Foda investigated 500 external rhinoplasty cases, and described a post-operative infection rate of 2.4 per cent.…”

“…The negative pressure arises from mucus and other secretions that progressively accumulate in the sinus and causes a gradual collapse in the orbital floor within weeks or months [3]. However, as it can be understood from the name of the syndrome, although the majority of patients have radiological sinus wall retraction and total or near-total opacification of the sinus; sinusitis and similar sinus pathologies are not observed [4,5]. Although the maxillary sinus is mostly affected in Silent Sinus Syndrome, other paranasal sinuses, including the frontal and ethmoid sinus, can be affected individually or combined fashion [2].…”
