Endotension: An Explanation for Continued AAA Growth After Successful Endoluminal Repair

Abstract: Excluded AAAs can increase in size owing to persistent or recurrent pressurization (endotension) of the sac even when there is no evidence of endoleak. One proposed mechanism is pressure transmission via thrombus that lines the attachment site. Endotension may also represent an indiscernible, very low flow endoleak that allows blood to clot at the source of leakage.

“…Compassionate proponents for EVAR have tried to repair failing endografts with numerous endovascular maintenance procedures in-stead of taking the risk of conversion to open surgery. When endovascular procedures have failed or are not possible, some patients have been left under mere observation (12)(13)(14)(15)(16).…”

“…39 Secondary interventions are typically performed when the aneurysm sac has become repressurized because of incomplete exclusion of blood flow from the sac. The term "endoleak" was created to describe this complication in 1996, 40 and a classification scheme has been adopted [41][42][43] (Figure 3). Type I and type III endoleaks are treated with immediate intervention to halt perigraft flow or flow between modular components.…”

“…However, there are reports of endotension patients who have undergone surgical conversion for aneurysm enlargement without any evidence of blood inside the aneurysm sac [20]. Pressure may remain high in the excluded aneurysm sac due to transmission of the pressure through thrombus at or around the ends of attachment zones of the prothesis [21,22]. This has been particularly associated with angulation of the proximal aspect of the graft [20].…”
