Endoscopic ultrasound-guided choledochoduodenostomy with a lumen-apposing metal stent through an uncovered metal duodenal stent

Abstract: Background and study aims  Endoscopic retrograde cholangiopancreatography (ERCP) in patients with a preexisting duodenal stent is particularly challenging and has a low success rate. Endoscopic ultrasound (EUS)-guided biliary drainage (EUS-BD) has been increasingly used as an alternative to percutaneous transhepatic biliary drainage after failed ERCP. EUS-guided choledochoduodenostomy (EUS-CD) and EUS-guided hepaticogastrostomy (EUS-HGS) have been reported to have similar efficacity. Recently, a novel dedicate… Show more

“…To our knowledge, only 2 similar cases are reported in literature; in both cases, the LAMS were released through the mesh of the duodenal SEMS such as in our patient. 19,20 Our report supports the use of EUS-CDS associated to SEMS placement in such challenging clinical scenarios. EUS biliary drainage is a salvage therapy that should be reserved for type I and type II bilioduodenal stenosis when ERCP is not feasible because of the presence of synchronous duodenal obstruction.…”

“…To our knowledge, only two case reports have been published in the literature about placement of a LAMS TTM of an uncovered duodenal stent after failed ERCP 27 28 . In our series, we experienced a 95.6 % technical success rate and no early or late AEs were recorded.…”

“…However, in such cases, patients have often undergone previous placement of a duodenal stent. To the best of our knowledge, there is only one case report of EUS‐guided biliary tract intervention after placement of a duodenal stent . Herein, we present the first case report of EUS‐GBD through the mesh of a duodenal stent for cholecystitis in a patient with pancreatic cancer.…”
