Emergency laparoscopy for abdominal stab wounds

Abstract: DL offers an important diagnostic tool in excluding peritoneal penetration in ASW, hence lowering the rate of unnecessary laparotomies. Given experience and skills, laparoscopy may be used therapeutically in selected cases of ASW.

“…Wound in the diaphragm may enlarge over time and lead to herniation of intraabdominal organs into the thoracic cavity, causing mortality in up to 48% of cases. [2,[8][9][10]17,18] Early diagnosis and treatment is essential.…”

“…[14,15,[21][22][23] Diagnostic laparoscopy is minimally invasive procedure that offers advantage of early detection and treatment of diaphragmatic injuries. [4,[6][7][8]17,21,24,25] Diaphragm injury rates have been reported in range of 10% to 50% in several studies that investigated left thoracoabdominal stab injuries using diagnostic laparoscopy. [1][2][3][4][5]8,22,26,27] In the present study, diaphragm injury rate was 23.5% in all patients and 22.1% in patients who underwent diagnostic laparoscopy.…”

“…11 A study estimating mortality and morbidity due to complicated diaphragmatic injuries reported rates of 20% and 30%, respectively. 17 The most common complication of a diaphragmatic injury is the herniation of abdominal content into the thorax which, if untreated, can cause complications that can lead to death. 17,18 Accordingly, ruling out violations or breaches of the diaphragm is crucial.…”

Unfortunately, non-invasive imaging modalities (CT and US) have been associated with high false-negative rates in the diagnosis of diaphragmatic injuries.…”

“…Diagnostic laparoscopy seems to have a potential role in the assessment of penetrating wounds, avoiding unnecessary laparotomy in many cases [43,44]. Nevertheless, the sensitivity of diagnostic laparoscopy is only 50% for evaluating kidney injuries [43].…”
