Toothbrushing programmes The BDA has backed new calls from NICE for oral health programmes in schools, calling on national government to support local authorities to turn the tide on an epidemic of tooth decay. NICE has recommended councils provide toothbrushing schemes in schools and nurseries in areas where children have poor oral health. Similar schemes exist in both Wales and Scotland, where devolved governments have set out dedicated oral health strategies that include outreach to early years and primary schools, and which have contributed to record breaking falls in decay. Despite progress by many local authorities , there is no equivalent programme in England. Health officials have claimed that devolution of powers to local authorities in England represents a barrier to rolling out a dedicated national programme. Objective The aim of this study was to examine the effect of Medicaid expansion on non‐traumatic dental condition (NTDC) emergency department visits in New York (NY) and New Jersey (NJ). Methods The 2010–2014 State Emergency Department Databases for NY and NJ were analyzed. NTDCs were defined as ICD‐9‐CM codes 520.0–529.9. Primary payers for ED discharges and patient's race were considered. Results In NY, from 2010 to 2011, there was a 51 percent decrease in private insurance and a 91 percent increase in Medicaid for NTDCs. In NJ, with the 2014 expansion, NTDCs fell 35 percent for uninsured and rose 57 percent for Medicaid. Black individuals have by far the highest population rates of NTDC ED visits, particularly in NJ. Conclusions The experiences in NY and NJ suggest that the timing of expansion had significant effects on payer distribution for NTDCs. Racial disparities continue exist with black individuals disproportionately accessing EDs for NTDCs.

ÖzetAmaç: Çocuklarda dental acil durumlara sıklıkla rastlanılmaktadır. Bu çalışmanın amacı antineoplastik tedavileri devam eden Akut Lenfoblastik Lösemi (ALL) hastası çocukların diş hekimine acil başvuru nedenlerini incelemektir. Yöntem: Çalışmada Türkiye'nin farklı coğrafik bölgelerinde ALL tanısı almış, Samsun'da farklı merkezlerde ALL tedavisi gören 32 çocuk ve ebeveyni dahil edilmiştir. Çocuklar farklı dişlenme aşamalarının görüldüğü yaşlara göre 3 gruba ayrılmıştır. Başvuru nedenleri şikayetin tipine göre; çürük, dental travma, oral patolojiler ve pedodontik apareylerle ilgili problemler olarak 4 gruba ayrılmıştır. Ebeveynlere çocuklarının diş fırçalama alışkanlığı, günlük şeker tüketimi, eğitim durumu, gelirleri ile ilgili sorular içeren ve bir anket doldurtulmuştur. Bulgular: Yaş ortalaması 5,8 olan çocukların % 55'inin süt dişlenme, %31'inin karışık dişlenme ve %14'ünün daimi dişlenme olduğu belirlenmiştir. Çocukların %74'ünün acil başvurusunun, ilk dental deneyimleri olduğu aileleri tarafından belirtilmiştir. Başvuru nedenlerinin sırasıyla pulpitise bağlı ağrı, gingival şişlik, fistül, yaygın selülit-ekstra oral şişlik ve travma olduğu saptanmıştır. Ebeveynin eğitim düzeyi, gelir seviyesi ile çocukların diş fırçalama alışkanlıkları arasında önemli derecede pozitif korelasyon bulunmuştur. Sonuç: ALL tedavisi gören çocukların tanı aldıkları anda çocuk diş hekimine yönlendirilmeleri, antineoplastik tedavilerinin aksamaması ve çocukların yaşam kalitelerinin düşmemesi açısıdan çok önemlidir.
