Elastotic nodules of the ear

Abstract: Eight cases of an elastotic degenerative lesion, developing on the ears in response to actinic damage are reported. Elastotic nodules may be diagnosed clinically as basal cell carcinoma, amyloid or even gout. Although usually found on the antihelix of the ear, they may arise on the helix and simulate chondrodermatitis nodularis helicis.

“…The outer root sheath (ORS) cells of the hair follicles are strong candidates given that BCCs are seldom observed in hairless regions of the body and appear to invaginate from structures resembling HFs (Weedon, 1981). BCC also has an immature, stem cell-like appearance leading to the suggestion that BCC originates from aberrant Shh activity within the stem cell population of the interfollicular epidermis or those of the hair follicle bulge (Taipale and Beachy, 2001;Zhang and Kalderon, 2001).…”

“…Mild superficial ulceration and a rolled pearly border on the anterior crus of the anthelix or the helix are frequently noted, and signs of actinic-related damage are most often present. [160][161][162] Histopathology. Histologic examination of the epidermis may show hyperkeratosis, mild basilar proliferation, increased pigmentation, and other signs of actinic-related damage.…”

“…Histological features include irregular coarse, elastotic material and clumped masses of thick fibers on a background of marked solar elastosis [6]. The fibers and clumps can be best seen with elastic-Van Gieson stain.…”

“…Otherwise, the other clinical patterns which are histologically similar are usually grouped together under the term ‘solar elastosis’ [4]. Solar elastotic syndromes include solar elastosis, Favre-Racouchot syndrome, elastotic nodules of the ears, collagenous and elastotic plaques of the hands and colloid milia [4, 5, 6, 7, 8, 9, 10]. Here we present two patients with a rare clinical presentation of solar elastosis in the papular form.…”
