Efficacy of colistimethate sodium as local application in necrotising fasciitis

Abstract: Necrotising fasciitis (NF) is a rapidly progressive severe soft tissue infection of the deep fascia resulting in the destruction of overlying subcutaneous tissue and skin. We report the case of NF of the lower limb with a poor prognosis due to multidrug-resistant (MDR) Klebsiella pneumoniae (K.pneumoniae) sensitive only to colistin. In view of the worsening condition of the wound, risk of deterioration of renal function and economic constraints, it was decided to start on colistin therapy locally by colistimet… Show more

“…Despite detailed instructions to maintain wound hygiene, the patient developed postoperative infection with Acinetobacter baumanii, sensitive to colistin and tigecycline. Local application of colistin instead of systemic administration, in susceptible infections, prevents renal toxicity and reduces the treatment cost [16]. In our case also, the infection was successfully controlled with local application of colistimethate sodium; however, the wound healed with scarring of lid margin.…”

“… 16 Also, there are reports of local application of colistin giving good results in MDR Klebsiella . 17 Hence, NPWTi with colistin instillation was considered for this patient. With three sittings of NPWTi, the wound granulated very well and the inflammation subsided.…”

“…There are only anecdotical reports on the use of topical antibiotics in NSTIs [119], as was suggested in necrotic pancreatitis [120]. Although their added benefit to parenteral treatment and surgery was not investigated, this approach does not seem suited as an exclusive first-line therapy.…”
