Behcet's disease (BD) is a multisystem mucocutaneous inflammatory condition characterized by recurrent genital and oral ulcers, ocular inflammation, and can involve the gastrointestinal tract. Treatment involves the usage of immunosuppressive agents to control the disease with glucocorticoids utilized for treatment of flares. Tumor necrosis factor inhibitors are frequently used to control the disease as well. We present the case of a 40 years old African American female presenting with intestinal BD that was refractory to adalimumab therapy. In conjunction with glucocorticoids, the patient's intestinal disease was controlled with infliximab therapy. Currently, there have been no studies comparing the efficacy of TNFα inhibitors on the treatment of BD. Future studies are needed to compare the efficacy of TNFα inhibitor agents in the treatment of intestinal manifestations of BD.

Behçet's disease (BD) is a type of vasculitis with many distinctive clinical manifestations and multifactorial immunopathogenesis. The cause of BD remains unknown, but it has been postulated that in a genetically predisposed or susceptible population, exogenous agents trigger the dysregulation of both autoinflammatory and autoimmune responses resulting in multisystem vasculitis. There are robust ongoing efforts across the globe to elucidate and identify signature markers to improve and assist in rapid diagnosis of the disease and to tailor the best therapy accordingly. While association of human leukocyte antigen (HLA)-B * 51 (B * 51:01 subtype) allele is well recognized as the strongest genetic susceptibility gene so far among genetically predisposed BD patients, further investigations using the latest technology have led to the identification of several novel single nucleotide polymorphisms (SNPs) and other associated genes involved in the pathogenesis. There are several "established" cytokines known to be involved in the pathogenesis of BD, which have been further implicated in the genome-wide association studies (GWAS)-based cytokine/receptor gene loci studies, as well as numerous "novel" cytokines, which are currently being studied and identified. This chapter offers insights into current knowledge and thoughts regarding the future of biomarkers in BD.

Behcet's disease is a multi-systemic, inflammatory, and chronic disorder characterized by recurrent oral aphthous ulcers, genital ulcers, uveitis, and other systemic organ involvement. Cardiac involvement in Behcet's disease is rare; however, it plays an important role in prognosis and increases mortality. The current researchers hereby have reported a case of Behcet′s disease with dilated cardiomyopathy. A 28-year-old male patient was presented with constitutional symptoms, oral and genital aphthous ulcers, pseudofolliculitis, tachycardia, arthritis, splenomegaly, erythrocyte sedimentation rate and C-reactive protein elevation, and left ventricular systolic dysfunction with left ventricular ejection fraction of 45%. Azathioprine and prednisolone were begun for the patient.
